Since 1970, Hale Kipa has championed Hawai'i's youth and children. We provide youth outreach, independent living facilities, therapeutic foster care, a haven for runaways, and more. Our founders recognized a need for a nonprofit to step in to assist this deserving population and we are still doing that critical work today, on O'ahu, Kaua'i, and the Island of Hawai'i. Role Overview
Hale Kipa is seeking a dedicated Case Manager to join our team, supporting, and providing case management services to our young adults in 4 of our residential programs: Transitional Living Programs (Men's and Women's homes), Independent Living Program Training Home (Men) and Haloa House (Women). The young men and women in our programs range in age from 17 ½ to 25. Some of the young adults are transitioning out of the Hawaii Youth Correctional Facility, transitioning out of homelessness and/or aging out of foster care. The Case Manager's duties entail: acting as a liaison to assigned staff and ensuring shift coverage, including covering Resident Advisor's shifts as needed; ensuring proper site maintenance; attending Facility Meetings; providing on-call coverage when the Program Manager is on PTO; being familiar with City and other housing projects for the targeted homeless population; assisting the Program Manager with placing young adults in appropriate housing programs; and being well versed in all eligibility requirements and benefits for young adults. Additionally, working with the young adults on independent living and social skill building, obtaining employment/vocational/educational resources, and any other services necessary to support the young adults in being successful contributing members of society. There may also be a need to attend court hearings for the Independent Living/Imua Kakou young adults.
The Case Manager also needs to be present at the homes, working in tandem with the Residential Support Staff as they manage crisis situations, provide a stable, safe, and nurturing environment, and help the youth/young adults work through their issues.
