The Hospice Aide provides compassionate personal care and assists in activities of daily living and other related activities to benefit the quality of a patient’s life. The Hospice Aide functions under the direction, instruction, and supervision of the Registered Nurse (RN) Case Manager. Documentation indicates following a plan of care and notifying the RN of changes in condition. Care for the patient and support of the caregiver(s) is provided where the patient resides. (patient’s home, facility, hospital, Inpatient)
Requirements:
Current Certified Nurse Aide training as per state law and Nurse Aide registry.
Minimum of two (2) years’ experience in direct patient care preferred. Ability to understand and carry out directions and instructions to maintain appropriate records and provide care as assigned.
High school diploma or equivalent.
Excellent organizational skills with attention to detail.
Demonstrated ability to work independently and in collaboration with other healthcare disciplines in a variety of environments.
Basic computer skills (Windows, Word) preferred.
Current CPR and First Aide certification as established by state law and accreditation.
Excellent communication skills (both written and verbal)
Ability to be flexible, problem-solve, seek advice, and educate others to processes.
Ability to answer the phone in a pleasant and helpful manner.
Reliable means of transportation with a valid state driver’s license.
