The Mather Evanston is Hiring an ASSISTANT DIRECTOR OF NURSING (ADON) Near Evanston, IL

We’re seeking an Assistant Director of Nursing to join our team at The Mather Evanston's on-site health center within our Life Plan Community. We provide 3 levels of care, including Medicare-certified Skilled Nursing, Assisted Living, and Memory Support. This role provides staff education, coordinates all MDS (minimum data sets) and care planning processes, and provides administrative leadership in partnership with the Director of Nursing (DON). Acts as nurse operations, regulatory and staff management leader in absence of the Director of Nursing.
Minimum Education, Skills, Knowledge, Work Experience:
Required: Minimum 2 years geriatric nursing experience; effective leadership, communication and presentation skills; current State of Illinois RN license; knowledgeable of related Federal and State nursing regulations; Able to problem solve and set goals with residents and customer- focused approach. MDS/PPS reimbursement experience.
Preferred: Experience with Memory Support and Dementia; Knowledge of Medicare Regulations; CPR/AED Certification.
Responsible for but not limited to:

  • Provides clinical oversight and supervision of all nursing staff in partnership with the DON.
  • Responsible for Assisted Living, Skilled Nursing and Memory Support operations.
  • Coordinates, monitors and manages the quality and outcomes of resident care.
  • Provides leadership, guidance and supervision for care venue staff members, including selection, engagement, recognition, performance management and corrective actions. Selects, directs, develops, educates, counsels and manages performance of staff in partnership with DON.
  • Supervises and coordinates nursing staff, ensuring staffing levels that support resident care activities.
  • Continuously monitors and evaluates staffing ratios and patterns in accordance with resident care and services. Responsible for the day to day operations of employee timekeeping and approval.
  • Assist the DON with education and mentoring of new staff and existing staff, and on-going individual, and regularly scheduled meetings and trainings.
  • Maintains regular communications with residents, family members and staff. Provides resident and family with education and counseling.
  • Coordinates MDS/Prospective Payment System (PPS) process, including educating and assisting staff to ensure the timely, accurate completion and transmission of MDS. Ensures that care plans and ICD10 coding are accurate and updated timely. Assures a safe environment through adherence to all Federal, State and MLW policies and procedures related to staffing, incident reporting, safety and risk management.
  • Monitors and measures Quality Indicators to ensure that Quality Assurance goals are met. Performs clinical and technical aspects of care according to established policies, protocols, standards of care and practice, regulatory mandates and within limits of the State Nurse Practice Act.
  • Attends QA/PI monthly. Identifies care center issues, proposes possible solutions and successfully adapts to changes as part of QA/PI process.
  • Participates in “on call” requirements, as applicable. Responsible to cover shifts as a staff nurse, when required.
  • Supports efficient operations of nursing department through hands on nursing and/or assuming administrative duties in absence of Director of Nursing.
  • Works with Assisted Living Manager to transition residents from Assisted Living/Memory Support to Skilled Nursing.
  • Assists in maintaining operating budget for resident care functions. Supports budget process and ensures operational efficiency accountability of the nursing care venues.
  • All other duties as assigned to support the operation of the Life Centre
