PROGRAM: Community Mental Health Center

POSITION TITLE: Clinical Assessment and Treatment Plan Developer

DEGREE REQ.: Master’s Level Clinical Supervisor

REPORTS TO: Clinical Supervisor

JOB TYPE: Full-Time/Part-Time

QUALIFICATIONS:

• Master's Degree required.
• Must be experienced in at least two of the following therapeutic services: psychotherapy, substance abuse, crisis management, psycho-education, and prevention techniques.
• Must have 4 years work experience in a non-profit setting and treatment environment.
• Must possess excellent verbal and written communication skills.
• Proficiency in the use of computers, software applications, and working knowledge of

Microsoft Word, Excel, and Outlook; willingness to learn EHR.

RESPONSIBILITIES:

• Provides comprehensive mental health assessments, treatment plans, and therapeutic treatments for clients.
• Assess clients using MH/LOCUS Assessment and IM CANS to recommend clients into the proper level of care.
• Create initial Treatment Plans for clients enrolling into treatment.
• Determines a client's appropriateness for services using the DSM-V.
• Participate on the Interdisciplinary Team for Care Plan Development which is composed of individuals who work together meet the physical, medical, psychosocial, emotional, and therapeutic needs of the agency’s clients.
• Must have excellent chart documentation and organization skills.
• Must be willing to adhere to the Counselor’s Code of Ethics and demonstrate a positive attitude regarding trauma informed care.
• Must have the ability to effectively work with Agency employees, outside contacts, and a diverse client population.
• Must have ability to de-escalate potential volatile situations between clients when necessary utilizing cognitive-based intervention strategies.
• Must be able to use appropriate forms and instrumentation to access client’s strengths and weaknesses. Develops, with the client’s participation, treatment plans to address identified problems.
