The Lakes of Clermont Health and Rehabilitation Center is Hiring a Social Services Director Near Clermont, FL
Overview
The Lakes of Clermont is conveniently located in the southern half of Lake County, within a mile of Orlando Health South Lake Hospital. 40 private and 20 semi-private suites will include individual bathrooms each with it's own shower. Enclosed, interior courtyards with water features will provide multiple locations for relaxing. For entertainment, a movie cinema on-site will provide residents with surround sound theater experiences. Personalized dining services will be provided by our trained culinary staff, including a registered dietitian.Responsibilities

- Identifying and providing for each resident's social, emotional and psychological needs, and the continuing development of the resident's full potential during his/her stay at the facility and to assist in the planning for his/her discharge.
- Managing the social services department personnel
- Participating in the pre-admission screening
- Providing information to families during admission
- Orienting residents to the LTC environment.

Qualifications
- Must have a Bachelor's degree in Social Work or license and/or certification to meet state and/or federal requirements for the position
- Basic understanding of computer technology including email use
- Ability to read, write and follow oral and written directions at a level necessary to accomplish the job
- Able to relate positively and favorably to residents and families and to work cooperatively with others
