Description

SUMMARY:

The Director of Care Coordination is responsible for oversight of care coordination operations at the assigned location(s). The Director is responsible for overall direction, analysis, monitoring, staff education and data reporting of the Institute’s care coordination programs.

REQUIREMENTS:

  • Direct supervision of care coordination staff including: care management coordinators, program director, nurse coordinators and care navigators/case managers.
  • Responsible for coordinating operations and billing/reporting with the Lead Health Home and Grantees
  • Oversee quality care management and case management services provided to all patients
  • Responsible for all reporting and generation of program and grant reports.
  • Coordinates partnerships with internal and external stakeholders.
  • Assists, develops and participates in quality assurance activities and promote high quality, comprehensive, effective patient care.
  • Oversees orientation of new program staff and complete orientation checklist.
  • Participates in and helps in the coordination of staff development programs and in-service trainings.
  • Manages all care management referrals, ensuring timely assignment and outreach.
  • Assists in the preparation and updating of policy and procedure manuals and health education materials.
  • Promotes interdisciplinary care of all patients

Qualifications


QUALIFICATIONS:

  • Master’s Degree in social work, public health, nursing or related field required. NYS licensure as LMSW, LCSW, PhD, PsyD, or RN preferred.
  • Experience in health homes or other Medicaid program strongly preferred
  • Computer literacy with proficiency in MS Word, Excel, and PowerPoint.
  • Demonstrated organizational, interpersonal, oral and written communication skills and the ability to handle multiple assignments at any time
  • Ability to generate, interpret and analyze data from multiple source
