Job Summary:
We are seeking a detail-oriented and experienced RN to join our team as a Home Health Quality Assurance (QA) Specialist. In this role, you will be responsible for reviewing and auditing clinician documentation to ensure accuracy, compliance with regulations, and adherence to best practices in home health care. The ideal candidate will possess strong clinical knowledge, excellent communication skills, and a passion for maintaining the highest standards of patient care through thorough documentation review.
Key Responsibilities:
• Review Clinician Documentation: Conduct regular audits of clinician documentation, including nursing notes, care plans, progress reports, and other patient records, to ensure accuracy, completeness, and adherence to company policies and regulatory standards.
• Compliance Assurance: Ensure that all documentation meets Medicare, Medicaid, and other payer requirements, as well as compliance with local, state, and federal regulations.
• Provide Feedback and Education: Collaborate with clinicians to provide constructive feedback on documentation practices and offer training or guidance when necessary to improve accuracy and quality.
• Quality Improvement: Identify areas of improvement in documentation processes and work with the clinical team to implement quality improvement initiatives to enhance patient care and compliance.
• Reporting: Prepare regular QA reports and summaries of findings, trends, and recommendations for senior management and clinical leadership.
• Policy Development: Assist in the development and revision of documentation policies and procedures to align with industry standards and regulatory changes.
• Collaboration: Work closely with clinical teams, including nurses, therapists, and case managers, to ensure that patient care is properly documented and that records accurately reflect the services provided.
Qualifications:
• Licensure: Active RN or LVN/LPN license.
• Experience: Minimum of 1 year of clinical experience in home health or a related healthcare setting, with at least 3 years in a QA or similar role.
• Knowledge: Strong understanding of home health regulations, including OASIS, ICD-10 coding (certification not required), and Medicare/Medicaid guidelines.
Skills:
• Excellent attention to detail and analytical skills.
• Proficient in electronic medical record (EMR) systems.
• Strong written and verbal communication skills.
• Ability to work independently and as part of a team.
• Certifications: OASIS certification or similar (preferred but not required).
