GuildCare, an innovative community-based Adult Day Health Care Program providing services to adults with health and vision needs is seeking an energetic and autonomous individual interested in part-time employment working 20 hours per week as an Outreach Worker for their Buffalo location. This individual will be responsible for promoting the GuildCare program, seeking out Medicaid-eligible clients in need of Adult Day Health Care services through visits and presentations at community centers, subsidized housing areas, healthcare agencies, etc. and obtaining appropriate referrals. A BSW or BS in Human Services preferred, ability to provide own transportation for local travel and computer proficiency required. Experience in outreach or marketing or case management preferred.
