Are you an experienced nurse looking for a rewarding opportunity in a skilled nursing facility? Look no further than The Grand Rehabilitation and Nursing at Pawling! We are actively seeking a dedicated RN to join our team.

This position is Full Time Monday-Friday with occasional weekend coverage as needed

Responsibilities :

• Conduct admission assessments for new residents and hospital returns, and develop initial care plans.
• Collaborate with the Director of Nursing (DON) to develop and implement strategies for the Nursing Department.
• Evaluate Patient Review Instruments (PRI's) and coordinate the admission process with the interdisciplinary team.
• Participate in discharge planning as needed.
• Assess and update residents' care plans based on changing conditions.
• Maintain accurate charting and documentation according to policies and procedures.
• Complete MDS assessments, care plans, and other required documents.
• Provide supervision, direction, and direct resident care as assigned.

Requirements:

• Active and unrestricted NYS Registered Nurse License.
• Prior experience as an admissions nurse in long-term care settings.
• Strong clinical assessment skills and ability to make independent decisions.
• Demonstrated leadership and supervisory abilities.
• Proficient in MS Office applications (Excel, Word, Outlook) and computer-savvy.
• Preferred experience in long-term care facilities.
• Excellent communication and interpersonal skills.
