Job Title: Registered Nurse - Home Health
Location: Hayward, California
Job Type: Renewable 13 Week Contract
- RN: Field position- Berkeley/ Oakland Territory
- Flexible schedule 9-5
- OASIS experience NOT required! Will train!
- 2 Years Home Health experience required
- Open to OT
- EMR: WellSky
- BLS/ CPR certification required
- CA License required
- COVID Vaccination required
- Mileage between patients- IRS Standard
We are seeking an experienced Registered Nurse to join our Home Health team on a renewable 13-week contract basis. As a Home Health RN, you will work collaboratively with other healthcare professionals to provide care to patients in their homes. You will be responsible for assessing patient needs, creating care plans, and providing education and support to patients and their families.
Responsibilities:
- Conduct initial and ongoing assessments of patient needs
- Develop and implement individualized care plans
- Administer medications, treatments, and procedures as ordered by the physician
- Educate patients and families on disease processes, treatments, and self-care techniques
- Communicate regularly with physicians, other healthcare professionals, and family members regarding patient care
- Maintain accurate and complete documentation of patient care
Qualifications:
- Valid and current RN license in California
- Minimum of 2 years of experience in Home Health or related field
- CPR certification
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Reliable transportation and valid driver's license
- Must be able to pass background check and drug screening
