Description

Tabor Hills Senior Living Campus, a not-for-profit community located in Naperville, IL, has been dedicated to caring for our senior population for 125 years. Tabor Hills Senior Living offers an Independent Living Community, Supportive Living Community, Senior Day Services and a Healthcare Facility (skilled nursing care, post-acute medical and rehabilitation, respiratory therapy.) Tabor Hills has a 5 Star Quality Rating by CMS.

$35.00-$39.00 per hour!

Location: Tabor Hills Healthcare Facility | 1347 Crystal Avenue | Naperville, IL 60563

RESPONSIBILITIES:

Listed below is an outline of the major duties and responsibilities. 

  • Assess resident’s care needs, strengths and potential, and plan interventions to maximize resident’s abilities to optimal level of function through the use of comprehensive assessment data base.
  • Plan nursing interventions for each resident based upon individual needs and monitor implementation of the approaches.
  • Monitor compliance with quality care standards, restorative protocols and local, state, Federal guidelines through documentation and auditing.
  • Administer direct resident care; provide treatments, administer medications, provide resident education and training, and oversee daily care routines.
  • Institute appropriate emergency interventions.
  • Make frequent rounds to evaluate effectiveness of care delivery and performance of subordinates to ensure compliance with quality care standards.
  • Ensure effective infection control practices and identify potential environmental hazards.
  • Assign resident care responsibilities and supervise nursing subordinates on scheduled shift.
  • Give report to CNA working in your assigned area.
  • Communicate with Power of Attorney and Physician about changes in resident condition, occurrence and discharge plans.
  • Promptly respond to resident calls and ensure that routine, special and emergency needs of all residents are met at all times.
  • Communicate with Director of Nursing on resident matters.
  • Provide direct resident care as necessary.
  • Communicate with physician during rounds. Keep physicians notified of any changes in condition, reactions/responses to medications, treatments or restorative programs, progress or lack of progress of care programs or incidents/accidents.
  • Sign out all medications and treatments at the time of administration.
  • Ensure conformance to established practices related to drug administration. Maintain controlled drug records and report any discrepancies to Supervisor immediately.
  • Attend and participate in scheduled in-service programs and staff meetings.
  • Report all personal incidents/accidents immediately to Supervisor.
  • Maintain confidentiality of necessary information.
  • Utilize supplies and equipment properly and without waste.
  • Think and act calmly and logically to meet unusual occurrences of the job without being thrown off stride.
  • Perform any miscellaneous work assignments, as may be required.

Job Qualifications


  • Valid Illinois Registered Nurse License.
  • Current CPR Certification required.
  • Ability to communicate with residents, staff and visitors.
  • PHYSICAL DEMANDS: Lifting 75 TO 100 pounds by self with frequent Lifting and/or carrying of objects. Exerting considerable force frequently and/or intermediate force constantly to move objects and residents. Reaching, handling, talking, hearing, and visual near acuity.
