Symphony Care Network is Hiring an Admissions Liaison Near Linden, MI

Symphony Linden, part of the Symphony Care Network, is growing their team and is hiring for an Admissions Liaison.

Admissions Liaison Responsibilities:

Internal:

· Work closely with the Symphony Centralized Admissions Department and the center on patient clinical and equipment needs/updates, status on transfer/admission ensuring the facility is prepared for the admission.

· Call prospective families to welcome them to Symphony, offer a tour and help them with the transition to our care.

· Conduct hospital on-sites for clinical review and to support quality transitions of care.

· Work closely with the Assisted Living Advisor on conducting tours, lead generation, and community events.

· Create and implement marketing events with the Assisted Living Advisor to encourage referral sources and the community to visit/tour our center/campus.

· Support customer service and first impressions initiatives.

External:

Acts as the liaison between the hospitals and the center.

· Preform pre-admission screens to ensure a safe transition of care.

· Responsible for account integration with the goal of generating referrals/admits and increasing market share year over year.

· Work closely with the Symphony Centralized Admissions Department and the center on patient clinical and equipment needs/updates, status on transfer/admission ensuring the facility is prepared for the admission.

· Meet with patients and families at the hospital bedside to answer questions and secure Caretel Inns Brighton as the facility of choice.

· Create and implement marketing events to encourage referral sources to visit/tour our center/campus.

Admissions Liaison Qualifications:

· Minimum of two years marketing and/or Liaison experience in Senior Living, SNF, LTC, Home Health or Hospice preferred.

· Must have the ability to establish good working relationships with health care professionals.

· Bachelor’s degree in nursing, healthcare, business administration or other healthcare related field preferred.

· This position reports to the center Administrator with support from the Vice President of Business Development.

· Seeking a self-directed professional with initiative who requires minimal supervision.

· Excellent observation, verbal, and written communication skills.
