Supportive Living Solutions has a growing Case Management program, and we are looking for an experienced individual who has a strong desire to help shape and grow the team! This is an exciting opportunity for you to leverage your administrative and leadership skills to have a greater impact in your career. The ability to make connections, build teams, and support a growing department are key to being successful in this role.

Job responsibilities include: 

  • Provide leadership to a team of up to 10 case managers
  • Build a positive team that is committed to achieving results
  • Manage team performance to meet or exceed efficiency metrics
  • Resolve client and service provider questions, concerns, complaints and requests in a timely manner
  • Create positive, solution-orientated relationships with stakeholders and providers
  • Ensure quality documentation through routine training and auditing
  • Maintain open balances below department goal

Schedule and Compensation: 

  • Full time, Monday Friday, daytime hours
  • Starting at $50,000 annually, negotiable based on education and experience
  • Benefit package including health, dental, life insurance, 401k and PTO (benefits accessible after 90 days of employment) 

Qualifications:

  • 2 years experience in waiver case management in Minnesota
  • Experience leading and developing a team
  • Technical aptitude and ability to train others on software systems
  • Ability to pass DHS background check
  • Valid Driver's License, proof of car insurance, and willingness to use personal vehicle in the delivery of case management services
