Pay: $25-$30/hour (LPN)| $35-$45/hour (RN)
Are you a Registered Nurse / RN or Licensed Practical Nurse / LPN with experience in geriatric care? If you are looking for your next opportunity, this is for you! Sunflower communities has an opportunity for hardworking and caring nurses to join our team! Successful applicants will have prior experience in caring for the older adult population.
Nurse - RN / LPN Overview:
- Oversee the coordination and plan of residents’ care, ensuring care practices have each resident’s best interests in mind
- Complete clinical assessments of residents’ health status and document appropriately
- Monitor the status of chronic illnesses and manage the administration of medications
- Coordinate with external health care providers and internal team membersInteract with residents’ families and loved ones, reporting changes in their health or care plan
Nurse – RN / LPN Qualifications:
- Licensed as a Registered Nurse / RN or Licensed Practical Nurse / LPN in Minnesota
- Experience with Electronic Medical Records (EMR) software
- Prior nursing experience in a SNF, AL, TCU, or Home Health setting is ideal
- Demonstrated ability to work autonomouslyExcellent at planning, organizing, and communicating
