We are Urgently Hiring a “Master Social Worker” for 3 Months of contract in Mount Pleasant, Wi.

Title: Master Social Worker.
Duration: 3 Months.
Shift: 8hr Day Shift (40)- 8am-4:30pm.
Required Certifications: MSW.
JOB DESCRIPTION
Provides social work services for health care related situations for patients and their families.
Collaborates with other healthcare providers to explore and initiate strategies to achieve specific desired outcomes.
Skills required: Discharge planning, care coordination, excellent communication with care management, physicians, and patients/families, prioritization, anticipation of needs.
Patient Types: Inpatient, M/S population.
Charting System: EPIC.
Weekend Rotation: every other Saturday.
Holiday Requirements: None.
Floating Requirements: None.
Job Types: Full-time, Contract
Pay: $45.00 - $50.00 per hour
Schedule:
- 8 hour shift
- Day shift
