Stanley Total Living Center is Hiring a Beautician Near Stanley, NC

1. Beautician will provide services in accordance with professional standards and all laws, rules, and regulations that may apply or relate to the performance of such services as outlined by the NC cosmetology board.

2. Beautician will provide the facility with a current cosmetology/beautician/barber license to be maintained on file in the main office at both STLC and The Parke at all times.

  • This license must also be displayed in each salon at all times, visible to customers.

3. Beautician must provide the facility with personal professional liability insurance coverage which is maintained and updated for the duration of the contract.

4. Beautician will provide services based on routine Medicare/Medicaid guidelines at STLC specifically—this includes a cut/shampoo every 6 weeks for those on Medicaid who desire such services. This also includes the inability to accept tips for services rendered under federal law.

5. Beautician will provide services to residents at The Parke in which tips can be accepted since the services for this facility are not paid for by Medicare/Medicaid.

6. Beautician will maintain records documenting all services provided in both buildings and will obtain staff or resident signatures as evidence of services rendered.

  • All records will be copied to the Business Office every 2 weeks for payment purposes.

7. No resident is to be left alone in the STLC salon at any time for any reason due to safety guidelines for nursing homes residents. Residents at The Parke can be left alone if necessary as this is independent living.

  • Breaks are to be scheduled around appointments.

8. Beautician will be responsible for providing all products and supplies necessary to provide services with the exception of towels/washcloths (see below provided by facility).

9. Beautician will be responsible for maintaining a clean, orderly salon at all times and will follow specific infection control guidelines for the industry. This will include prior to leaving the salon at the end of every day.

  • State survey inspections will be performed routinely and are unannounced.

Beautician is responsible for maintaining the shop in an orderly, safe manner that will pass each inspection without concern. Issues with a state inspection may result in cancellation of the current contract without notice by the facility.

10. At STLC, beautician will be responsible for getting residents from their specific unit for their scheduled appointment and then for returning them back to their specific unit upon completion of their scheduled appointment.

  • The beautician must notify nursing staff of the appointment upon removal from the unit and upon return to the unit for safety purposes.
  • Nursing staff will be responsible for transferring a resident to and from the wheelchair to the beauty shop chair and vice versa at ALL times for the safety of both the resident and the beautician when services cannot be done in their personal chair.

At The Parke, residents will come directly to the shop for his/her own scheduled appointments without staff assistance.

11. Beautician will be expected to follow all facility policies and guidelines in terms of infection control, dress code, etc. as provided (see attached).

  • Facility will provide an annual contract services training annually (July) as required by the Centers for Medicare & Medicaid Services for review. All procedures outlined in this training must be followed as written.

12. Beautician will introduce himself/herself to each new resident personally and each

Responsible Party via telephone within the 1st week of admission, regardless of whether the resident/family have chosen to utilize the services of the salon. If the resident/family desire services, this meeting will be to (a) set up the initial appointment for services and (b) begin the development of a resident profile for preferences/choices in terms of hair. If services are not requested at that time, the meeting is simply to begin a rapport with both the resident and family, so they feel comfortable if they choose to request the services in the future.

13. Beautician will be responsible for handling any concerns or complaints made by residents or families regarding services.

14. Beautician will not allow any non-licensed person in the shop at any time for safety purposes.

15. Beautician will be responsible for setting the hours of each salon based on the needs of the residents and scheduled appointments at any given time. This includes holidays, evenings, and weekends.

  • Appointments at STLC will be made around meals so that residents are not interrupted and able to eat each meal as necessary.
  • The Administrator will be made aware of any scheduled time off/planned vacations as well as any unexpected absences which will result in closure of either salon with as much notice as possible for communication purposes.

DUTIES OF THE FACILITY

1. Facility agrees to provide space for an appropriate salon at each location and to maintain such space and equipment in accordance with industry practice.

2. Facility will provide routine housekeeping services inside the shop routinely.

3. Facility will provide laundry services for facility towels/wash cloths used for salon services.

  • Beautician will go to laundry to pick up clean laundry prior to the 1st appointment each day and will then drop off dirty laundry at the end of each day for laundering purposes.

4. The facility will maintain an active NC license for each salon, paying all salon fees as required.

5. Facility will be responsible for all monies (cash or check) from services rendered in the shop. Any resident or family wishing to pay for services will be directed to the Business
Office by the beautician. NO monies (cash or check) with the exception of allowed tips from residents at The Parke will be handled by the beautician for any reason.

6. Facility will notify beautician of all new admissions as well as any new service requests.

7. Facility will pay 80% of all sales from each salon to beautician every (2) weeks—the 20% to the facility will be considered to be the booth rental fee.

  • As an added incentive to keep a minimum volume limit of no less than $550 of service provided during a 2-week period—the facility will pay 85% of all sales to beautician (15% booth rental fee).
  • The method of payment will coincide with the same schedules as employees are paid (every 2 weeks). Billing sheets must be turned in to the Business Office no later than the Friday prior to pay day Thursday.

8. Facility will review pricing for services at least annually and will make any changes that are comparable to other facilities in the local area as necessary.

TERMS AND CONDITIONS
The term of this agreement will commence on the _____ day of _______, 20____ and will continue for (1) year as the initial term. This agreement will automatically be extended for an additional (1) year period upon the expiration of the initial term and will continue annually unless either party notifies the other in writing not less than (30) days prior to the expiration date of such term.

If the beautician or the facility fails to keep, observe, or perform any material covenant, agreement, term, or provision of this agreement, either party may terminate this agreement without written notice.

In performing the services hereunder, both parties acknowledge and agree that the beautician is acting as an independent contractor (self-employed) and not as the agent, partner, or employee of the facility. Neither party has the authority to bind the other to any third party or otherwise to act in any way as the representative of the other. As an independent contractor, the beautician is not exclusively limited to performing services for the facility and is entitled to perform services for other parties outside the facility.

FACILITY POLICIES/PROCEDURES
FOR THE BEAUTICIAN

1. Personal Appearance

Inappropriate clothing includes:
--Sundresses, halter tops, backless dresses, tank tops, or plunging necklines
--Midriff tops, spandex, or see through clothing
--Shorts of any kind
--Colored or patterned designs underneath clothing which may appear though clothes
--Baseball caps or hats
--Long, dangling jewelry that may be unsafe for the resident or the employee
--Jeans

Dress should be business casual with the exception of Friday, which is casual day—jeans may be worn on that day only. There are also other days throughout the year that are considered “dress up days” in which the dress code will be announced (hat day, pajama day, tacky day, etc.)

2. Telephone Use

The telephone is provided for the transaction of business only. Cell phones are not to be used while on duty for privacy purposes. Pictures may also not be taken of any resident without his/her expressed permission (or that of his/her family member) based on the facility abuse policy.

3. Confidentiality/Privacy

Any and all resident information is confidential. Medical records, resident conditions, and statements made by and about residents are confidential at all times. Discussing confidential matters freely, inside or outside the facility, will result in immediate cancellation of this contract for services and reporting will be made as required by law.
Any individual known to relay personal information about our residents knowingly or unknowingly could suffer penalties including fines by the state as well as termination of employment.

4. Parking

Parking in spaces closest to the building should be reserved for facility visitors. Contract staff are to park in the 400 unit lot, from the employee parking sign in the 100 unit lot all the way around to the back of the building, or at the First Presbyterian Church.
