The Case Manager offers support to individuals with SPMI and SUD in a manner that is strengths based and supportive. Daily activities focus heavily on engaging with residents in a home like setting to develop the skills necessary to transition into a more independent living setting. Activities focus on relationship building, and group activities. The ideal candidate would be motivated, compassionate, and able to work independently. Other primary activity are to assess the needs of residents and the household and support an environment that fosters growth.

Functions:

• Develop and maintain of the social service program 
• Assist in providing support to residents in the evening at the direction of program manager or Case Manager II
• Oversee meal preparation, and other communal activities 
• Maintain accurate and timely records which include, completion of a daily log, case notes, checking and sending emails, and assist in other paperwork to ensure chart conformance
• Understand and in a supportive manner enforce house rules and expectations
• Monitoring the facility inside and out and assess for safety issues

Skills/Requirements

• 1 years experience in Social Work or related field
• Clinical Dependency Counseling/Mental Health experience preferred
• CCDC or CCDC II certification preferred
• PSS Certificate preferred
• Good problem/conflict resolution skills
• Good verbal/written communication skills
• Good decision-making skills
