Description:

Breast Patient Navigator- FT M-F 8hr shifts

POSITION SUMMARY: The Breast Patient Navigator (BPN) serves as a single point of contact during initial and follow-up phases of breast health care among referring physicians, patients, and caregivers. The Navigator provides resources and assistance in accessing clinical and supportive care services offered within the Breast Program continuum and in the community. Serves as a care coordinator ensuring seamless and timely coordination of services throughout the continuum of breast health (breast imaging, surgery, medical oncology, radiation oncology, plastic surgery, and genetics). Identifies patient and family needs and facilities access to the services required to meet those needs. Meets with patients and their families at the onset of diagnosis and plays an integral part in each patient’s management. Works as a team member and utilizes the nursing process to provide patient-centered care and disease management. Serves as the primary point of clinical contact for all patients requiring services. Works with clinical providers as a physician extender to optimize patient satisfaction and patient care processes.

EDUCATION REQUIREMENTS:

Active NY RN License

OCN Oncology Nursing Certification.

BREAST NAVIGATOR CERTIFICATION/OR BCRN Certification preferred (Must achieve certification within first year in role)

EXPERIENCE REQUIREMENTS:

Minimum 3 years or greater cancer care experience and/or specialized breast services.

Advanced knowledge of current screening, diagnostic, treatment options and community resources in cancer.

Problem solving, decision making and critical thinking, excellent leadership, organizational, written, and verbal

communication. Excellent interpersonal and presentation skills.

Must be able to work independently as well as in teams.

Computer literate; Microsoft Office competency required
