St. John's United is Hiring a Care Coordinator Near Billings, MT
Summary:
The Care Coordinator will follow the Wraparound process to provide care coordination to identified children, youth their families. The C.A.R.E.S. Wraparound model is a strengths-based, family driven diversion program designed to divert youth and families from the child welfare system, juvenile justice system or residential care.:
Essential Duties and Responsibilities:
- Facilitate Family Team Conferences and follow the Wraparound process to manage caseload
- Build relationships with family members, providers and the community
- Promote family voice and choice in work with families and community providers
- Practice trauma informed care
- Complete all necessary documentation to ensure Wraparound model is followed with fidelity
- Perform other incidental or related duties as assigned:
- Bachelor’s degree in social work or related field
- Knowledge of mental health systems of care and support services in Yellowstone County
- 2 years of experience working with children, youth and families that have complex needs and are multi-system involved:
- Excellent written and verbal communication interpersonal skills
- Demonstrate cultural competence sensitivity to population served
- Proven ability to work with diverse groups and build strong partnerships
- Support and uphold the mission, beliefs and values of St. John’s United
- Proficient in the use of computers and Microsoft office programs and virtual web platforms:
- Candidate must have valid driver’s license, proof of current insurance and be able to drive throughout the state of MT. Other national travel may be required.
- Must be able to pass a thorough background screening upon hire.
