Springpoint at Home is seeking a Social Work Care Manager, Part Time to join our team. This positon will report to our Monroe Township, NJ office for trainings and monthly team meetings. The Social Work Care Manager is a health and human services professional who acts a s a guide and advocate for their clients and/or families. The Care Manager will maintain a specialized focus on issues related to aging and elder care. Duties will include evaluation of patients/clients and/or families and oversee planning and problem solving, education and advocacy, and provide family caregiver coaching. Responsibilities: Promotes client psychosocial wellbeing and self-determination.Coordinate with doctors, family members, trust officers, accountants and other members of an older adult's support network.Provide and/or arrange for transportation to/from medical appointments, social events and other personal outings.Completes assessments, reports, and other documentation as required.Submits to Springpoint at Home Facilitator all client billing hours every month or as requested.Assumes responsibilities when necessary to assure client/family needs are met.Education/Experience: Graduate degree in Social Work or Masters in Social Work, Gerontology or Counseling. Two (2) to Four (4) years of experience in geriatric care management preferred. Psychiatric background and training is a plus. Proficiency in Microsoft Office. Strong organizational and time management skills. Ability to handle multiple tasks and meet deadlines.This position primarily covers Mercer-Middlesex Counties.
