Clinical Coordinator (LVN) 

LVN:Licensed Vocational NurseJob Type: Full- time

QUALIFICATIONS:

  •  Must be a graduate of an accredited school of practical/vocational nursing
  • Current licensure to practice vocational nursing in the State of Texas by the Board of

Nurse Examiners and must maintain licensure throughout employment.

  • LVN, 2 years (preferred)
  • Experience using a physician EHR system is preferred
  • CPR & Basic Life Support (BLS) certification required

FULL JOB DESCRIPTION:

We are seeking an LVN to join our team as a Clinical Coordinator. This is an office position working to review clinical information to coordinate care for patients needing home healthcare. The majority of this coordination is done through use of a computer or other electronic based communication, applicants must have knowledge of basic computer use. This coordination is vital to our team ensuring that our patients receive the care they need acting as the bridge between the patient, the field staff and the providers. We value a patient-focused, development-oriented and cohesive team. We are looking for a nurse who is passionate about clinical excellence and has the ability to work independently and as part of a team.

ESSENTIAL FUNCTIONS & RESPONSIBILITIES:

  • Must also have a strong understanding of medical terminology and excellentorganizational skills in order to ensure that the needs of all patients are being met
  • Ensure H&P is present on referrals and review/understand orders
  • Strong organizational and communication skills required
  • Answering telephones
  • Acting as a liaison between patients, patient families, and health care professionals ensuring their needs are being met and resolve any issues that may arise
  • Updating patient records to the electronic medical record system
  • Coordinate with office and field staff to meet patient care needs 

PHYSICAL DEMANDS:● Normal vision (or corrected vision) and hearing.● Ability to type for extended periods of time● Ability to tolerate viewing on computer monitors for extended periods of time.
