Title: Clinical Case Manager Division: HOPICS- Intensive Case Management Services (ICMS)

FLSA: Non-Exempt, Full-time Supervisor: Program Manager

Pay Range or Rate: $22.00 - $25.00 per hour Revised: 10/4/2022

Summary

Clinical Case Managers provide field-based intensive case management services inclusive of but not limited to homeless, mentally ill, and persons experiencing substance use disorders. The Clinical Case Manager will provide direct services to assigned clients and coordinate care by assessing clients' needs and facilitating access to appropriate resources. Assessment will include but not be limited to acuity level of client support and engagement, biopsychosocial assessment, service provision, crisis management, and triage.

Essential Functions

  • Be familiar with and able to document in accordance with the Department of Mental Health guidelines (general standard model used).
  • Provide screening/intake assessments and address emergent/immediate needs of clients including but not limited to the need for medical care, shelter, food, clothing, and employment to determine specific service needs.
  • Develop and implement an Individual Service Plan which addresses client needs and goals including basic needs or harm reduction, if applicable.
  • Update this Plan periodically to reflect clients' progress towards or attainment of articulated goals and the emergence of client needs.
  • Develop and maintain positive, collaborative relationships with community service providers including chemical dependency treatment providers, mental health providers, landlords, and detox centers.
  • Enter data into the CHAMP system within 24-48 hours of client contact
  • Provide structured Intensive Case Management Services consistent with SSG/HOPICS and program policies.
  • Assist clients in scheduling and keeping appointments.
  • Advocate for the client with community partners and a wide variety of other service providers.
  • Assist clients in gaining entry into service programs.
  • Identify gaps and barriers in available community resources and advocate for systemic changes.
  • Attend Case Management staff meetings and other required meetings.
  • Develop and maintain client files and other necessary reporting for assigned caseload according to contract requirements and any additional duties as assigned.
  • Required to report (verbal/written) in accordance with the law all legal issues that surface (Tarasoff, child abuse, vulnerable adult abuse, danger to self, or gravely disabled)
  • Represent the Agency in a professional manner at meetings and community events
  • Experience working with external entities such as courts, Los Angeles County Law Enforcement Agencies, District Attorney Offices, and Victims Advocate Representatives, the Department of Children and Family Services, and other community agencies.
  • Maintain regular attendance
  • Maintain files/records on client services in compliance with HIPAA & CFR42 and other funding requirements for auditing purposes.
  • Other duties as needed.

Minimum Qualifications - Knowledge, Skills, and Abilities Required

  • Master’s Degree from an accredited college or university in Social Work or a related field and Two (2) years of job-related experience working with homeless individuals and families OR five years of experience working in the social service field; case management and homeless program experience preferred.
  • If in recovery, a minimum of three (3) years of being drug and alcohol-free is required.
  • Knowledge of resource development, case management, and documentation.
  • Ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds.
  • Ability to communicate effectively, both written and orally.
  • Verification of Employment Eligibility and Background Clearance.
  • TB test is required every year or as needed.
  • CPR and First Aid Certification are required every two years or as needed by the company and a valid Driver’s License and auto insurance are required.
  • Reliable transportation is required.
  • Ability to communicate effectively, both written and orally.
  • Experience working with homeless families and children, DCFS as well as experience with placing homeless families into permanent housing.

Additional Requirements

  • Computer literate, with basic knowledge of Microsoft Office Word, and Excel as well as a high level of initiative in keeping current with technological change.
  • Ability to prioritize workload and activities of self and complete tasks in a timely and efficient manner.
  • Dependable, able to work under pressure; receptive to change, willingness to learn, cooperative approach to problem-solving.
  • Ability to establish and maintain effective working relationships with staff, clients, and outside contacts from a wide variety of ethnic, socioeconomic, and cultural backgrounds, and good diplomatic skills.
  • Good basic clerical skills, including typing, ability to proofread own work, good grammar, and spelling.
  • Flexible team player.
  • Excellent attention to detail.
  • Ability to work very independently and seek supervision when appropriate.
  • Ability to set boundaries, resolve conflict, and de-escalate issues.
  • Must be able to pass a comprehensive criminal background check.
  • Possession of a valid California Motor Vehicle Operator’s License (local travel on behalf of the Agency may be required utilizing company vehicles).

Language Skills

  • Ability to read and interpret general business correspondence, policies and procedures, referral information, financial information, financial documentation, and applicable government regulations.
  • Ability to write case notes, uncomplicated reports, instructions, and procedures.
  • Ability to present information effectively and respond to questions from patients, staff referral sources, and the general public.

Mathematical Skills and Reasoning Ability

  • Thorough knowledge of and ability to apply business arithmetic skills accurately and rapidly
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where standardization may be limited. Ability to interpret a variety of instructions furnished in written, oral, schedule, or diagram format.
  • Basic math skills.

Physical Requirements

  • The employee is regularly required to sit; use hands to finger, handle or feel objects, tools, or control; reach with hands and arms and talk or hear; frequently required to stand, walk, and kneel; occasionally to climb, balance, or stoop; rarely to crouch or crawl. The position also requires driving personal and company vehicles.
  • The employee must occasionally lift and/or move up to 30 pounds.
  • The employee must have close, color, and peripheral vision and the ability to adjust focus.
  • The noise level in the work environment is moderated.

Mental Requirements

The incumbent to be able to perform effectively under any and all of the following

  • Unpredictable situations, crises, constant distractions, interruptions, uncontrollable changes in priorities/work schedules; s/he is able to process information, think, and conceptualize.

Environmental Conditions (Working Conditions)

This position is responsible for working in “client-friendly” environments and is required to visit other shelters and homeless access centers as part of their duties. Local automobile travel is required. There is some responsibility to work in noisy environments where children and adults are present.
