40 Hour Case Manager needed for Southeast Kansas Aging and Disability Resource Center. Requires a four-year degree from an accredited college or university with a major in health, social work, education, human services, or related area, or a Registered Nurse licensed to practice in the State of Kansas. Position involves completing phone and in-home assessments for persons who need home services or nursing facility placement. Primary assignments will be in Allen, Neosho, Woodson Counties, reliable transportation needed, mileage costs reimbursed. The person in this position will need good communication, paperwork, and organizational skills, and computer skills. A background check is required. Please submit resume and cover letter.
