JOB SUMMARY

Sky View Rehabilitation and Health Care Center, a 192-bed Skilled Nursing Facility located in Croton-on-Hudson, has openings for Full-Time Occupational Therapists. New grads welcome! Duties include, but are not limited to:

• Initiates OT services upon physicians orders and in accordance with the Resident’s Comprehensive Care Plan.
• Conducts necessary evaluation and/or re-evaluations as ordered and determines the rehabilitative goals for each Resident based on the Resident's need relative to their physical and mental level of functioning, overall care plan and the Resident's preferences.
• Determines each Resident's treatment goals which shall range in a continuum, progressing from all specialized restorative rehabilitation services to routine maintenance rehabilitation.
• Monitors the maintenance of optimal levels of functioning for those Residents who have been discharged from the formal rehabilitation program.
• Interprets evaluation findings to patient, family and care team.
• Develops treatment plan including goals and methods to achieve identified goals and coordinates with patient, family and care team under the direction of referring physician.
• Participates in care conferences, quality assurance and other committees as assigned.
• Implements treatment plan directly or supervises treatment by a COTA.
• Monitors patient response to intervention and modifies treatment to attain goals.
• Supervises and assigns appropriate activities to COTA’s, and Rehabilitation Aides.
• Documents results of patients assessment, treatment, follow-up and termination of services according to the department policy and procedures, maintains all service related records.
• Assesses the need for equipment, supplies, and other similar items for the department. Reports repairs and maintenance needs as per facility policy and procedure.

Requirements

•  Must possess a current NYS License in Occupational Therapy which is in good standing with the NYS Board of Education.
• Long term care experience a plus!
