Job Title: Case Manager
FLSA Status: Full Time, Exempt, Regular
Work Schedule: 40 hours per week
Department: Senior Health and Wellness (Dept# 100)
Reports to: Director of Coordinated Care
Pay: $55,000-$57,000 Salary
Benefits: Comprehensive benefits, PTO, Sick Leave, and 401(k) available to full-time employees starting on the 1st day of the month following 30 days of hire.
GENERAL PURPOSE OF JOB
Silver Key’s Case Management team serves clients of various ages, incomes, abilities, medical needs, and diagnosis. The Case Manager’s role is to provide whole person, person-centered wraparound services that address the 8 dimensions of wellness, which include: Environmental, Emotional, Financial, Social, Spiritual, Occupational, Physical, and Intellectual wellness. This is achieved through care planning and goal setting and determining appropriate referrals to internal and community-based programs. Focuses range from short-term to long-term services that meet basic needs, address client emergencies, and foster systems navigation. The case manager must maintain a high level of knowledge about Silver Key and ensures services and programs remain compliant and deadlines are met. All roles are expected to consistently embody the Silver Key values and strive to meet the high expectations of Silver Key’s brand, mission and vision.
ESSENTIAL DUTIES AND RESPONSIBILITIES
• Meet with clients in various settings including but not limited to the office, home, tele-health, facility, hospital or other appropriate locations, as needed.
• Document in SOAP format and linking medical needs to the billable services being provide
• Coordinating and providing care that is safe, timely, effective, efficient, equitable, and client-centered
• Handling case assignments and provide ongoing support and expertise through comprehensive assessments, planning and care plan implementation to meet client needs, review case progress and determine case closure
• Helping clients achieve wellness and autonomy while demonstrating a professional demeanor.
• Facilitate multiple care aspects (case coordination, information sharing, etc.) and develop effective working relations and cooperate with medical team throughout the entire case
management process and advocate for clients within Silver Key and outside
agencies/organizations.
• Help patients make informed decisions by acting as their advocate regarding their wellness status and treatment options.
• Arrange meetings with social services, health, and governmental agencies.
• Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction
• Promote quality and cost-effective interventions and outcomes.
• Assess and address motivational and psychosocial issues.
• Uphold ethical and procedural social services agency requirements.
• Maintain client information and services confidentiality.
• Participate in programs implementation improvement with co-workers and support coworkers
through information sharing and collaboration.
• Participate in community outreach and education which may include facilitating psychoeducational groups.
• Work independently and as a team member.
• Volunteer engagement
• Provide on-call weekend and evening services on a rotating basis.
• Other duties as required.
VOLUNTEER ENGAGEMENT RESONSIBILITIES
Silver Key is a volunteer centric organization. You may be regularly interacting and engaging
with volunteers, so it is your responsibility to show appreciation for their efforts and
occasionally provide guidance.
DRIVING DUTIES
Will occasionally be required to drive on behalf of Silver Key Senior Services. Must maintain a
current, valid Colorado Driver’s License. Must maintain the state issued minimum automotive
liability insurance coverage. Must observe and obey all federal, state, and local traffic laws.
Responsible for the proper function and maintenance of any Silver Key vehicles used in
performing assigned duties.
EMPLOYMENT STANDARDS
Silver Key Senior Services is a drug-free organization. Employment is contingent upon
successful completion of a drug screen and background checks. Business-specific knowledge:
Knowledge of Silver Key Senior Services mission statement, policies/procedures, business specific
software, and safety regulations. This position does require a background check by Colorado Adult Protective Services (CAPS).
REQUIRED EDUCATION and/or EXPERIENCE.
Requires a bachelor's degree from a four-year accredited university in a human services-related
field plus two years of experience working with seniors within a social service or clinical setting.
DESIRABLE, PREFERRED, OR SPECIAL QUALIFICATIONS
Master’s degree is preferred. Familiarity with care planning, SOAP/DAP documentation formatting,
and diverse payor sources. Excellent written and verbal skills. Proven working experience in case
management, including as a Medical, Mental Health Care Manager or a related job.

• Either supported or supports an older adult directly in navigating behavioral health services
LANGUAGE SKILLS
Ability to read, analyze and interpret data, professional journals, technical procedures, or
governmental regulations. Ability to effectively present information and respond to questions from
groups of managers, clients, customers, and the public. Regular use of client-centric language as
well as ability to pick up non-verbal cues. Ability to have culturally sensitive communication and
practice de-escalation. Ability to be compassionate and maintain professional boundaries.
Proficiency in non-English languages is highly valued.
MATHEMATICAL SKILLS
Basic arithmetic and ability to interpret data including percentages and business rates.
REASONING ABILITY
Ability to solve complex problems and deal with a variety of abstract variables in situations where
only limited standardization exists. Ability to interpret a variety of instructions furnished in written,
oral, diagram and or schedule form. Ability to speak with clients, actively listen to gain
understanding of their needs, and decipher solutions. Candidate must be able to demonstrate
independent critical thinking.
CERTIFICATES, LICENSES, REGISTRATIONS
• Must possess and maintain a Colorado Driver’s License (or obtain one in 30 days of hire).
PHYSICAL DEMANDS
This position requires standing, walking, and sitting for periods of time. Utilization of hands and
fingers to operate office computer keyboard and equipment required. Must be able to reach
with hands and arms, talk, hear, and stoop, kneel or crouch if needed. Must be able to lift up
to 30 pounds as needed. The Case Manager will be required to drive to meet clients in various
settings.
VISION REQUIREMENTS
Position requires close, distance and peripheral vision.
WORK ENVIRONMENT
Moderate noise within business office atmosphere with computers, printers and light traffic.
Other workspaces may include community outreach locations, client homes, and shelters with
differing work environments.
