Home Health Registered Nurse

In this role, you will have the opportunity to provide high-quality, personalized care to patients in the comfort of their own homes.

Responsibilities:

• Conduct patient assessments and develop individualized care plans in collaboration with the interdisciplinary team.
• Administer skilled nursing care to patients in accordance with physicians' orders and established policies and procedures.
• Provide wound care, medication management, and other nursing interventions as necessary.
• Educate patients and their families about their conditions, treatment plans, and self-care techniques.
• Monitor patients' progress and adjust care plans as needed to promote optimal health outcomes.
• Communicate effectively with patients, families, and other members of the healthcare team to ensure continuity of care.
• Document all patient interactions, assessments, and interventions accurately and in a timely manner.

Qualifications:

• Current license to practice as a Registered Nurse in the state
• Minimum of one year of clinical nursing experience, preferably in home health or a similar setting.
• Strong assessment, critical thinking, and communication skills.
• Ability to work independently and prioritize tasks effectively.
• Reliable transportation and valid driver's license.
• Willingness to travel to patients' homes in the Portland area

• Full-time status guaranteed as long as the 20-patient caseload per week is met.

• Competitive hourly rate with potential for increases based on experience and performance.
• Starting at $58/hour (rate can increase based on experience)
