10330884- RN Home Health - San Ramon, CA - 13 Weeks Contract
Sigma Healthcare teamis looking for experienced Registered Nurse Home Health to work at our client's facility in San Ramon,CA.
Shift: M-F, days
Summary:
- Responsible for traveling to patient's homes or skilled nursing facilities in a localized area around the regional office to assess and perform skilled nursing services. As well as document assessments, actions and outcomes.
- Evaluate patients and develop appropriate care plans
- Apply care plans according to individual patients
- Assess outcomes and manage care plan modifications as needed (documentation through OASIS software)
- Provide nursing services based on the care plan devised (G-tubes, J-tubes, Trachs, wound care, etc.)
- Supervise nursing assistants in their everyday work such as bathing and grooming
- Administer medication as instructed
- Manage emergency procedures
- HomeCare HomeBase/Pointcare experience required
- At least 2 years of experience in Home Care as a RN
- Currently BLS certified
- Valid RN license (where applicable)
