Service Access & Management, Inc is Hiring an Early Intervention Service Coordinator - Part Time Near Chambersburg, PA

HOURS: M-F Approximately - 25.5 - 30 hours per week

RATE: $19.00 PER HOUR

Summary of Responsibilities:

Under the general direction of the Early Intervention Supervisor, this person provides Service Coordination for children and their families. Services are provided in accordance with all existing SAM policies, SAM standards of operating/established procedures, state/federal regulations, and also in accord with the mission and values of the Company.

Essential Functions:

  • Request and/or complete authorizations for service
  • Assist in identifying and accessing community resources and providers
  • Verify MA status/insurance eligibility
  • Complete and update service plan within SAM required timelines
  • Complete referral to appropriate agencies and/or units within SAM
  • Gather prior treatment information
  • Provide crisis intervention
  • Provide support to persons served/family
  • Participate in inter-agency planning
  • Attend court hearings
  • Complete written reports
  • Maintain accurate and up-to-date files
  • Complete case notes within required 48 hours of the visit
  • Complete release of information processes
  • Participate in discharge/transition planning
  • Complete required training(s)
  • Prepare files for closure
  • Attend all scheduled meetings
  • Provides oral and written translation for Spanish-Speaking Consumers

Essential Functions for Early Intervention Service Coordinator:

  1. Gather data to complete an initial referral for Early Intervention Program
  2. Schedule Multi-Disciplinary Team Evaluation (MDE) with family
  3. Complete Ages and Stages screening tool
  4. Conduct the Individualized Family Service Plan Process (IFSP)
  5. Facilitate development of transition plan to preschool services
  6. Meet frequency of SAM contact standards
  7. Achieve SAM productivity standards
  8. Meet SAM required timeline of 48 hours for case note entry
  9. Meet SAM required timelines for multidisciplinary evaluation
  10. Meet SAM required timelines for development of IFSP
  11. Meet SAM required timelines for transition planning

Personal Characteristics:

  • Comfortable performing multi-faceted projects in conjunction with day-to-day activities
  • Ability to get along with diverse personalities; be tactful, mature, flexible
  • Possess sound judgment and reasoning abilities
  • Possess oral and written communication skills
  • Be resourceful and well organized
  • Comfortable with the team process
  • Ability to establish credibility
  • Be decisive, assertive and persuasive 

Minimum Requirements: (education, experience, etc.)

  • A Bachelor’s Degree from an accredited college or university which includes 12 college credits in Early Intervention, Early Childhood Special Education, Early Childhood Education, Child Development, Special Education, Family Counseling, Family Studies, Social Welfare, Psychology or other comparable Social Sciences, and 1 year of full-time or full-time-equivalent experience working with or providing counseling to children, families or individuals with disabilities.
  • Computer skills to include Microsoft Office
  • Pre-Employment Drug Test
  • Automobile
  • Act 33 and Act 34 Clearances
  • FBI Fingerprint Clearance
  • Immigration and Naturalization (I-9) Clearance

Required Licenses, Certificates or Knowledge:

Valid Pennsylvania Driver’s License and acceptable MVR

Working Conditions:

In-office work performing concurrent, varied tasks

Daily travel throughout Franklin/Fulton County areas to providers and other human service agencies, as well as occasional travel to statewide meetings

Safety Hazards of the Job:

Working hours spent predominately in the community must, therefore, have awareness of personal/community safety. There may be a rare occasion during which it is necessary to have physical interaction with a consumer exhibiting aggressive behavior

Some travel may occur during adverse weather conditions
