Summary of Responsibilities:

Under the generaldirection of the Unit Supervisor, this person is responsible for providingsupport coordination services to individuals assigned. Using helping skills anda strengths-based approach, this person builds a trusting rapport withindividuals to better understand their unique needs, interests, and theirvision of a good life. This person provides ongoing monitoring, support, andlinkage to available systems and supports within the community. SupportsCoordinators promote the philosophies of Everyday LivesValuesin Action, and Self Determination within service delivery.

Essential Functions:

Lead teams to supportthe people we serve; Collaborate with individuals, families, and providers toensure people are living their best lives; Facilitate meetings and discussions;Advocate; Critically think and problem solve; Links individuals to communityresources, funding, and services; Assess needs; Create plans; Document serviceprovided; Empower individuals in choice and the right to lead an EverydayLife.

Education and Experience:

Bachelor’s degree,which includes or is supplemented by at least 12 college credits in sociology,social welfare, psychology, gerontology, criminal justice, or other relatedsocial science, or two years’ experience as a County Social Service Aide 3 andtwo years of college-level course work, which includes at least 12 collegecredits in sociology, social welfare, psychology, gerontology, criminaljustice, or other related social service, or any equivalent combination ofexperience and training which includes 12 college credits in sociology, socialwelfare, psychology, gerontology, criminal justice, or other related socialservice and one year of experience as a County Social Services Aide 3 orsimilar position performing paraprofessional case management functions.

Employment Requirements:

Automobile and ValidInsurance, Valid Pennsylvania Driver’s License, acceptable Motor Vehicle RecordCheck, Act 33 and Act 34 Clearances, FBI Fingerprint Clearance, Social SecurityCard, Immigration and Naturalization (I-9), Drug Screening Clearance.

Working Conditions:

Prolonged periods workingon a computer in an open office environment, comfortable communicating throughvirtual technology, and travel throughout the assigned region may occur and maybe in adverse weather conditions, must be able to lift up to 15 pounds
