SECTION I: JOB PURPOSE

Work with health plan members to introduce Community Supports Services and complete the Opt-in process for CS Services.

SECTION II: KEY RESPONSIBILITIES

Core Responsibilities:

  • Assess program members at program entry to jointly develop a strengths-based, goal-driven action plan that promotes permanent housing and self-sufficiency in the future.
  • Identify members' strengths and barriers to stability to help ensure proper housing and resource options.
  • Provide comprehensive case management support to program participants and ensure services are individualized, culturally competent, family-focused, goal-driven, strengths-based, and appropriate.
  • Match program members to appropriate housing and resource options, always taking their needs and ability to generate income into account.
  • Build relationships with program members to learn their needs, goals, strengths, and barriers to maintaining stable housing; keep the channels of communication open, and be proactive about problem-solving.
  • Maintain records and notes for each member, track and report members' outcomes in excel worksheet and organization database, and provide weekly and monthly progress reports to the supervisor.
  • Conducts comprehensive screening ad assessments with individuals members or families to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information, as appropriate to help them locate and maintain housing and break the cycle of homelessness.
  • Build a feasible, sustainable Individualized Housing Support Plan based on the Community Support Assessment that addresses barriers and includes measurable goals.
  • Prepare program members for eventual self-sufficiency:Assess barriers the member faces

Build connections with potential employers and training programs.
Prepare members for job interviews; assist with building a resume.
Teach members budgeting and financial literacy skills.
Ensure members possess basic life and home care skills; provide training, if and when needed.

  • Identify and provide outreach to landlords and community organizations to educate them about Community Supports and related services: Locate suitable Community Supports resources and housing options.

Build relationships and connections with landlords, social workers, case managers, administrators
Serve as a landlord liaison to help mitigate any issues that arise with tenants.
Work with tenants and landlords to solve issues and prevent eviction.

  • Conduct visits in members homes once housed to ensure compliance with the lease, rules, regulations, and program agreements.
  • Conduct in-person visits at members community location to ensure compliance with set Community Supports goals.
  • Coordinates case conferences with residents, consults with service team, monitors progress of clients on case plan, assists clients to meet the agreed upon goals
  • Attend team meetings and training

SECTION III: KNOWLEDGE & SKILLS

  • Associates Degree level or equivalent (In social services related discipline)
  • Or a combination of education and relevant experience (1 year total)
  • Wide and deep knowledge of Microsoft and Google
  • Proven leadership skills with the ability to motivate and inspire staff
  • Strong interpersonal skills and the ability to add value to any project/initiative
  • Has track record of introducing innovative ideas and solutions into business
  • Strong financial management and planning skills
  • Capability to perform under circumstances of possible emotional stress and conflict, including dealing with uncooperative members and families.
  • Ability to communicate effectively in oral and written English language.
  • Active listening skills
  • Demonstrable skills in advocacy
