Overview:
The Care Coordinator's primary responsibility is to facilitate Child and Family Planning Meetings using wrap-around principals that are family-centered, individualized, strengths-based, and outcome-focused. Monterey County Wraparound’s goal is to help children and their families with complex and enduring needs receive intensive treatment in the least restrictive environment that is appropriate. The facilitator’s role is to provide assessment of the client and family’s needs and lead the team in providing treatment to support the family toward meeting their goals.
About Seneca Wraparound Programs
The focus of Seneca Wraparound programs is to offer family-centered, individualized, culturally relevant, and strengths-based support to families with complex and enduring needs. Wraparound works in close partnership with Department of Social Services, Juvenile Probation, Behavioral Health, and group homes. Wraparound provides strengths-based and outcome-oriented alternatives to group care placements for children and youth by drawing upon community and family strengths. Treatment and support services are tailored to address behaviors and meet family needs, even as they change over time. At the core of the Wraparound model is the Family Team, comprised of the child/youth, family members and other adults connected with the child/youth, the County representative who referred the child/youth for Wraparound, and Seneca staff including a Facilitator, Support Counselor, and Family Partner. The Wraparound team builds consensus and leverages the strengths of its members to identify and meet needs that require intervention to support the child/youth and family in reaching their long-term goals.

Responsibilities:
- Facilitation of Child and Family Planning Meetings.This includes prepare for the meeting, coordinating attendance, facilitate the meeting, write meeting notes and distribute them within two days, write the mental health notes, and clean up after the meeting
- Case management and coordination of care
- Collaborate with county and community partners including but not limited to county social workers, probation officers, outside therapists, psychiatrists, teachers, etc.
- Travel up to 90-mile service radius
- Utilize a cultural lens
- Complete comprehensive assessments, treatment plan of care, mental health notes, Family Team Meeting notes, Wrap Action Plan, incident reports, and other documentation as necessary and in a thorough, accurate and timely manner
- Intervene with clients’ behavior management programs and provide crisis management as needed. In rare circumstances when there is serious threat of harm, this may include physical management of the clients (implementation of Seneca Center’s Equilibrium Model Training)
- Maintain accurate documentation and paperwork including, but not limited to the following: time sheets, petty cash, and mileage
- Attend team meetings, weekly trainings, individual and group supervisions and adhere to clinical best practices
- Maintain a flexible schedule that includes early mornings, evenings and occasional weekends
- Participate in providing emergency on call response as scheduled (typically 24 hours at a time, three times per month)
- Maintain professional behavior at all times, appropriately representing Seneca’s mission, values and philosophy
- Function as a program leader and uphold the ideals of the program
- Perform all other duties as necessary for the good of the agency as directed by your supervisor
Qualifications:
- Bachelor’s Degree
- Must be eligible for MHRS certification (4 years of clinical experience in a mental health setting)
- Must have demonstrated competence, reliability, and good writing skills in clinical documentation.
- Demonstrated competence in the skill areas of positive client engagement, communication, and an understanding of the challenges that families involved in the Child Welfare and may be experiencing.
- Experience working with a diverse population.
- Ability to work as part of a multidisciplinary team.
- Willing and able to work flexible hours.
- Own Vehicle, Valid California Driver’s License, and Insurance.
- Obtain and maintain fingerprint clearances with the DOJ, FBI and Child Abuse Central Index.
- Proof of Covid-19 vaccination and Booster required.
Schedule:
- Full-time
- Monday - Friday
- On-call shifts required
