Position Summary
: The position will provide personal care service to the terminally ill patient and perform related tasks in accordance with the plan of care as assigned and delegated by the RN-Patient Care Coordinator (PCC).


Duties Include
:

  • Provide personal care to the patient in a compassionate and understanding manner consistent with the hospice philosophy.

  • Provide personal care to the patient at the written and/or verbal direction of the RN.

Qualifications Include:

  • Graduate of a state approved nursing assistant training program

  • Current, active certification on the Minnesota Nursing Assistant Registry; current BLS for Health Care Providers or will obtain within 3 months of start date

  • Valid driver’s license with acceptable driving record per policy

  • Knowledgeable of Medicare Hospice rules and regulations as well as Minnesota Standards of Practice
