Job Summary

The RN of Adult Day Health Care Program (ADHCP) is responsible for coordination and implementation of all medical services provided to registrants of the program.

Qualifications


A. Education and Training – Currently licensed and registered to practice as a Registered Professional Nurse in New York State. New graduate nurse may be considered. Current cardiopulmonary resuscitation (CPR) certification issued by either the American Heart Association or the American Red Cross. Current driver's license.

B. Experience - Minimum of one year nursing experience in a long term care setting is preferred.

C. Character - The individual must possess moral character, patience and compassion for the elderly.

III. Essential Job Functions

A. Read, write, speak, hear, reason and remember.

B. Stoop, kneel, crouch, reach, stand, walk, push, pull and lift heavy weight (greater than 50 lbs.).

C. Observe and assess registrant’s condition and act accordingly keeping all information in strictest confidence. Supervise people and create good relationships. Supervise people and create good staff relationships. Must keep all information in strictest confidence.

D. Must be willing and able to perform basic life support (BLS) at all times, including CPR, the Heimlich maneuver and rescue breathing.

E. Perform all job duties and responsibilities as outlined in IV.

IV. Job Duties and Responsibilities

The RN of ADHCP is supervised by the clinical Director of ADHCP. The job duties and responsibilities are to be performed while observing the following standards: job standard (knowledge, ability, skills), technical skill, quality, quantity, judgement/problem solving, decision making, and initiative. The responsibilities include, but are not limited to the following:

A. Coordinate services for registrants with services provided by the community or other agency programs including, but not limited to, long term home health care programs, certified home health agencies, social services agencies, clinic, and hospital outpatient departments and services.

B. Ensure that duplication of services to individual registrants is avoided.

C. Supervise activities of all personnel to ensure that registrants receive assistance in accordance with their needs.

D. Secure the services of the registrant's personal physician or alternate when indicated.

E. Accept responsibility for providing or arranging inpatient and/or other services for those registrants for whom the program is no longer suitable.

F. Arrange transportation services for registrants.

G. Interview and assess potential registrants as directed by Director of ADHCP.

H. Supervise, direct, and instruct medical needs of ADHCP staff.

I. Provide or arrange for all nursing services to registrants.

J. Maintain registrant's complete health record.

K. Maintain and promote good rapport with all in-house departments.

L. Maintain good public relations with outside organizations and facilities, always representing the program in a professional manner.

M. Acts as ADHCP Supervisor in absence of Director/Clinical Director. Attend meetings as required.

N. Coordinate and conduct health care education for registrants/staff.

O. Complete all incident/accident reports.

P. Follow all regulatory requirements in regards to the program's operations.

Q. Complete and maintain individual registrant care plans.

R. Documentation for nursing rehab program.

S. Supervise registrant meal time.

T. Performs other related duties as required by their manager.

V. General Duties and Responsibilities

A. Participate in all department meetings, mandated inservices, and assigned committees.

B. Maintain a positive and professional relationship with registrants, family members, legal representatives and visitors.

C. Maintain a positive and professional relationship with all personnel, including supervisors, co-workers and volunteers.

D. Follow all safety regulations and precautions at all times; use protective devices as required.

E. Wear and maintain proper uniform/clothing as established in the uniform dress code policy, including name tag identification. Maintain acceptable personal hygiene.

F. Maintain confidentiality and registrant rights.

G. Maintain confidentiality of all issues and subject matters, including records, reports, data and other related sensitive materials.

H. Maintain an acceptable attendance rate.

I. Follow all laws, rules and regulations of the Federal and NYS Health Codes in regard to the department and overall organization operations.

VI. Working Environment

The program RN is subject to inside and outside environmental conditions. The RN may also be exposed to odors and fumes. Works in a private office, ADHCP room and throughout the facility as needed and in the community when assessing for admissions. Hours of work are from 7 a.m. to 7 p.m. Overtime hours may be encountered. The program is open Monday through Friday from 7 a.m. to 7 p.m. and Saturday 11 a.m. to 4 p.m.

VII. Exposure to Blood and Airborne Pathogens

This position has been categorized into Occupational Safety and Health Administration (OSHA) Category 1: routine contact with blood/body fluids of persons living or non-living. Contact can be made while assessing registrants, performing personal care, completing treatments, handling regulated medical waste and in clean up of spills and other blood/body fluid accidents. Protective equipment should be utilized when performing these tasks.
