SCAN INC is Hiring a Remote Case Manager

Case Manager - Community Partners
Under general direction of a Program Supervisor, complete screening and assessments of new clients and coordinate with appropriate service providers. Provide weekly case management for families engaging in services. This position requires the ability to work independently, exercising judgment and initiative.

ESSENTIAL FUNCTIONS
  • Visit individuals in homes to complete assessment and intake process, provide information on agency services and connect families to supportive services for their needs.
  • Visit individuals in the home after assessment and provide ongoing weekly case management providing hands on services and connecting to resources to prevent child abuse and neglect.
  • Serve as initial point of contact for program services, including client outreach for families referred by outside agencies.
  • Maintain caseload as assigned, which may include ongoing case management work.
  • Assist with client transportation and crisis assistance as needed, such as hands on lice treatment.
  • Serve as client advocate and respect all agency and program confidentiality policies.
  • Report any case that may involve “imminent harm” or “threatened harm” to a supervisor immediately.
  • Complete all paperwork required by the program within established time frame and use program database and agency equipment to complete documentation and other job duties.
  • Develop and review service plans in consultation with clients and coordinate hand off visits with partner agencies.
  • Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.
  • Travel is expected for this position, including daily travel within the region assigned to complete client visits and occasional travel within the state for required trainings and meetings. The assigned region (DCS Region 4) includes Allen, Whitley, DeKalb, Noble, LaGrange, Whitley.
  • Performs other duties as assigned.

  • Demonstrate motivation and ability to work with professionals involved in working on behalf of clients.
  • Understanding of and empathy for clients served, including those of different cultures, races, ethnic identities, and special needs.
  • Ability to collect and maintain confidential and detailed client information in a timely fashion.
  • Ability to communicate effectively, verbally and in writing, with a diverse range of people, including clients and staff at a level necessary for satisfactory job performance.
  • Needs to have reliable daily dependable transportation to use on the job and to transport others. Must have valid driver’s license and proof of insurance
PHYSICAL DEMAND
  • While performing the duties of this job, the employee is regularly required to talk or hear and listen.
  • Climb and maneuver steps in narrow hallways and in small rooms.
  • Tolerate strong odors.
  • Lift and physically manipulate up to 25 pounds.
  • Climbs stairs/flights of stairs
  • Read printed material in poorly lighted areas.
  • Operate common standard office equipment.
  • Discern and evaluate sounds, interactions, and movements in multiple situations to determine the potential for child abuse and neglect.
  • Use senses of sight, hearing, and smell to evaluate the safety of the environment for children.
  • May be subjected to infectious diseases, substances, and contaminated articles (bed bugs, lice, etc.).
  • May be exposed to animals while at family’s home.
  • Sit for extended periods of time.
EXPECTED HOURS OF WORK
  • Some flexibility in hours is allowed, but the employee must be available between the "core" work hours of 8am to 8pm Mon- Fri with occasional weekends to meet program and agency needs. Staff must work 40 hours each week to maintain full-time status.
REQUIRED EDUCATION
  • Bachelor’s Degree in Social Work or related human service field or Bachelors’ Degree in other fields with a minimum of three years’ experience working directly with the families in a human service field.
PREFERRED EDUCATION EXPERIENCE
  • Previous experience working with children and families.
  • Previous successful case management experience.’
  • Experience with Microsoft Word, Excel, and Windows
