Summary:

This position reports to the Direct Services Program Manager; The Case Manager’s primary focus is to address the specialized case management needs of participants in housing programs in coordination with additional agencies and resources to address the client needs through referral and in providing assistance with benefit and identification assistance. The Case Manager is to help families secure permanent housing, income, and other additional resources to become self-sufficient for long term stability and may perform duties that include outreach, prevention and risk reduction, crisis intervention, health education, referral for substance abuse and mental health counseling, and benefits counseling. The Case Manager works with administrative staff dedicated to various grants, to ensure program funds are used according to grant requirements with support and guidance from direct supervisor.

There is a $1,000 Hiring bonus for candidates hired to be paid the first half at the end of their completed 90-days of employment and the second half paid at end there six months of employment.

Case Management:

  • Assess client readiness for case management services based on needs and ability and willingness to participate in the program.
  • Create case plans for clients with short, medium, and long-term goals and the steps to achieve those goals.
  • Assist in developing savings and budget plans in preparation for eventual independent living, including the development of housing and benefit enrollment applications.
  • Prepare case-related reports including outcomes, successes, and challenges.
  • Provide relevant referrals to agencies and service providers based upon the goals and objectives mutually agreed upon with the client.
  • Advocate for clients with agencies and service providers to minimize barriers to clients receiving needed services.
  • Follow up with referrals to ensure that clients receive and access necessary services in a timely manner.
  • Establish and collaborate with the community to establish and maintain referral resources, educate the community on the STF programs.
  • Record, enter, and monitor client data per contract requirements and ensure all necessary outcomes are being met and program compliance are being followed.
  • Maintain and complete organized client files (with 95% accuracy as documented by STF file audits), compile and produce reports and analyses of program effectiveness as requested and enter data into HMIS (with 95% accuracy per HMIS audits) and other computer data base systems.
  • Maintain detailed timekeeping and ensure accurate client information is kept to reflect budget spent down per grant requirements.
  • Provide a customer service focused effort to work with the public, volunteers, clients, and other staff members as needed.
  • Perform additional related duties as required.

Qualifications:

  • Possess at minimum an A.A. degree or bachelor’s degree in a related field, preferred. 
  • Has relevant work experience in non-profit management, case management and/or program development, preferred.
  • Has experience with social services, housing /homeless programs.
  • Possess the ability to use computers and proficiency in Microsoft Office software, and or the ability to learn new systems software.
  • Is a highly motivated self-starter, with proven ability to develop creative solutions.
  • Possess excellent written and oral communication skills, interpersonal skills, and ability to work both individually and as part of a team.
  • Able to maintain regular attendance.
  • Able to multi-task and remain calm in demanding and unpredictable situations.
  • Able to maintain a professional, customer service-oriented attitude always.
  • Able to meet face-to-face with clients of diverse backgrounds and disabilities, including physical, medical, mental and or substance abuse.
  • Able to develop rapport, provide information, and refer with sensitivity to individual differences.
  • Display a high level of initiative, effort, and commitment towards completing assignments efficiently.
  • Possess excellent time management skills and the ability to work with minimum supervision.

Requirements:

  • Must be over the age of 21, required.
  • Successfully complete an employment eligibility verification and background screening.
  • Possess or upon employment obtain a level one fingerprint clearance card.
  • Possess reliable transportation, valid driver’s license, and auto insurance.
  • Pass a Drug screening test – including the use of Marijuana.
  • Possess or upon employment obtain CPR/First Aid Certification
