Job Title: Social Worker
Department: Social Services
Reports to: Administrator
SUMMARY
Provide medically-related social services to attain or maintain the highest practicable physical, mental, and psychosocial well-being of each resident.
ESSENTIAL DUTIES/ RESPONSIBILITIES
·Other duties may be assigned
·Works in close collaboration with other health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying patient’s health problem.
·Helps patient and family through individual or group conferences to understand, accept, and follow medical recommendations.
·Helps patient and family to adjust to any changes, i.e., room change, new room-mate, new policies, death in family, etc.
·Provides service planned to restore patient to optimum social and health adjustment within patient’s capacity. Documents this plan through assessments, progress notes, patient care plan, etc.
·Utilizes community resources to assist patient to resume life in community or learn to live within limits of disability.
·Provide discharge planning services for patients no longer needing the services of a nursing facility.
·Assist patients with financial and legal matters, if needed.
·Prepares patient histories, discharge plans, and progress notes on all residents
·Maintain a waiting list on possible admissions.
·Communicate with Director of Nurses or the Case manager all necessary information for potential residents to determine appropriateness of admission to facility.
·Responsible for making certain that all family, residents, responsible parties have a thorough understanding of all payer sources available- Medicaid, Patient Liability, Medicare, Medicare co-insurance, secondary insurance, and private pay.
·Responsible for making certain that all new admissions are fully informed of his/her financial responsibility to the facility for care provided.
·Conducts Patient Rights In-service to employees
QUALIFICATIONS

EDUCATION/ EXPERIENCE
Bachelor’s degree from 4 year college or university with a degree in social work or in human services field such as sociology, special education, rehabilitation counseling, psychology and one year experience in a health care setting working directly with individuals
LANGUAGE
Ability to read, analyze, and interpret general professional journals, technical procedures, or governmental, regulations. Ability to write reports, correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, patients, patient families, and the public.
MATHEMATICAL SKILLS
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent.
REASONING ABILITY
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems quickly and efficiently involving patients, families, and staff.
