Job Description:
Social Services Assistant
Department: Social Services
Supervisor: Social Services Director
FSLA: Non-Exempt
Purpose of position: To assist in providing services to meet the social and or emotional needs that affect the residents’ ability to achieve their highest level of function; participate in the development of the residents care plans; assist with development of policies and procedures to provide social services to residents in compliance with federal, state and local regulations.

Position Responsibilities:
· Assist in implementing social service policies and procedures, meet residents’ needs in compliance with federal, state, and local regulations
· Assist in completing assessments for MDS (Minimum Data Set)
· File paperwork on charts and in social services office as requested
· Notify residents and families of upcoming care plan meetings
· Assist with visits from outside physicians such as dentists and podiatrists
· Assist in explaining residents’ rights to residents and residents’ representatives
· Assist with resident admission paperwork as assigned
· Schedule and implement room visits for residents who are unable to leave their rooms
· Provide assistance with discharge planning process
· Utilize community agencies/providers for resident no longer needing skilled care
· Complete progress notes on residents
· Provide education/materials of payer sources or assistance available to the disabled or elderly
· Assist with the grievance program
· Assist residents in obtaining personal needs
· Perform other related duties as assigned

Physical demands: While performing the duties of this job, the employee is regularly required to talk or hear. The employee may occasionally lift up to 50 pounds. Vision abilities include close vision, distance vision, peripheral vision, color vision, depth perception, and the ability to adjust focus.
Work environment: While performing the duties of this job, the employee may be exposed to blood borne pathogens, body fluids, infectious diseases, air contaminations and hazardous chemicals. The noise level in the work environment is usually moderate.
