Description

Position Summary

A community health worker (CHW) serves as a link between Sanitas medical centers and the community surrounding them. The CHW will be in charge of connecting with local resources, mapping the community and linking leaders and services to our users. During the workday, community health workers normally split their time between the assigned medical center and being on the ground in the area of the medical center to meet with individuals and organizations in the local community.

Essential Job Functions

  1. Identifying community resources at the local level in the areas surrounding the assigned medical center by being able to connect on the ground with different organizations, leaders and local clubs i.e. park activities, walking groups, knitting club.
  2. Connecting with organizations at the local level that can offer and organize community activities to set up strategies together.
  3. Creating and promoting group activities needed for the community that should be done both within our centers and outside our centers to have a better outreach, tackling issues and targeting interests of our communities such as loneliness i.e. Talking Café, physical activity, themed day activities (elderly)
  4. Facilitating self-sustained groups with specific communities identifying possible leaders of these groups both within our patients and in the overall community of our medical centers and helping them promote and set up other groups that can work on their own.
  5. Identifying community connectors key actors in the community that can help their community to connect to resources, identify needs, train them, organize periodical meetings with them to understand what are the major concerns and needs in the community and how many community referrals are happening on the ground.
  6. Supporting from a community standpoint those patients with highest needs by doing specific work with them to connect them to resources when they need extra help to do so.
  7. Work with the team at the medical center and participate in their weekly meetings to make sure the community approach is being done throughout the center.
  8. Report to the Community Coordinator in Florida.

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

While performing the duties of this job the employee is regularly required to work standing up, walk, use hands to operate tools and equipment and must be able to exert regularly up to 10 pounds of force, frequently exert 30 pounds of force and occasionally exert 50 pounds of force to constantly perform the essential job functions. The employee will be frequently required to reach with hands and arms, bend, balance, kneel, crouch, crawl, push, and pull. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus. The employee is subject to environmental conditions; protection from weather conditions but not necessarily from temperature changes. The worker is subject to noise; there may be sufficient noise to cause the worker to shout in order to be heard above ambient noise level.

Qualifications


Required Education and Experience

  1. Empathy and compassion for the clients we serve
  2. High school or equivalent and certified as CHW in Florida.
  3. 1-year experience in community development and/or community outreach.
  4. Communityrelation management knowledge of a localcommunitys needs and the ability to interview others to get needed information are essential qualities
  5. Public speaking
  6. Interpersonal skills
  7. Administrative skills
  8. General knowledge of the following software: Microsoft Office Suite (Outlook, Word, Excel, and PowerPoint)
  9. Bilingual Spanish and English required.

Preferred Education and Experience

  1. Relevant or any other job-related vocational coursework.
