What Will you be Tasked With?

  • Provides patient care as directed on aide care plan, reports and documents observations and care performed.
  • Observes and reports changes in client’s condition to Supervising Clinician.
  • Assists the patient in ADL’s (Activities of Daily Living) as directed on aide care plan.
  • Does not discuss patient or patient’s information outside of the Agency – maintains confidentiality
  • Communicates patient’s supply needs to Supervising Clinician or Health Care team.
  • Displays professional behavior at all times.
  • Complies with all applicable local, state and federal regulations.
  • Complies with all areas of corporate compliance program.
  • Documents thoroughly and accurately in format prescribed by the agency.
  • Attends In-Service Training programs to maintain currency in practice and certification.
  • Must comply with HIPPA regulations as they pertain to PHI and assorted patient information.
  • Perform related duties as assigned.
  • Demonstrate compliance as they relate to HIPPA.
  • Provide companionship and patient care as directed on care plan.
  • Provides Assistance in personal grooming and dressing as directed on patient care plan.
  • Provides a showering or bed bath for non-ambulatory, semi-ambulatory and ambulatory patients as directed on care plan.
  • Measures and performs vital signs with proper documentation if included in care plan.
  • Performs full patient transfers, patient turning and assistance in ambulation as directed on care plan.
  • Provides meal planning, preparation and serving as directed on care .
  • Provides feeding assistance as directed on care plan.
  • Provides linen changing techniques for ambulatory and non-ambulatory patients as directed on care plan
  • Practices infection prevention and control measures on all care provided.
  • Provide toileting techniques for ambulatory and non-ambulatory patients.
  • Observe patients physical, mental and emotional conditions and report any changes to the supervisor.
  • Help patients and their families understand instructions given by medically trained staff.
  • Perform Safe transfer patients as directed on care plan.
  • Perform related household tasks as directed on the care plan

Environmental Conditions and Physical Requirements

  • Positions in this class are typically situated in a home environment. This position may be required to go out into the field alone to perform patient care. As a result there will be exposure to all types of weather, from snow to rain to temperatures in excess of 100° Fahrenheit. The ability to gain ingress and egress at all types of locations (e.g. homes, apartments, mobile homes and other domiciles is essential.)
  • Position routinely performs cooking, transfers, toileting and bathing. These actions require the mobility to lift patients, stoop, bend and reach.
  • Driving in a motor vehicle, standing, sitting, walking, bending, reaching and stretching.

Minimum Requirements

  • Able to provide two verifiable references.
  • Must have vehicle, current driver’s license, and appropriate automobile insurance coverage.
  • Minimum age of 21.
  • Minimum 1-year experience as a Home Health Aide.
  • High School Graduate with valid diploma.
  • Current certification as a Certified Home Health Aide by the State of California required.
  • Current CPR card.
  • Documentation to verify work experience and legal right to work in the U.S.
  • Reports to work in a timely manner and adheres to call-in times for schedules changes.
  • Regularly participates in case conference, in-service training and corporate compliance programs, as required.
