Visit Rates are as follows: Admission Evaluation/Resumption of care with Oasis: $130.00 per visit, Discharge with Oasis: $110.00 per visit without Oasis: $90.00 per visit, Recertification/Re-evaluation: $110.00 per visit, Evaluation without Oasis: $110.00 per visit, Routine Visit: $90.00 per visit, Telephone Discharge: $20.00, Hourly rate for orientation, training, and meetings: $40.00.
