Rivercross Hospice is Hiring a Hospice Patient Care Manager Near Muskogee, OK

Rivercross Hospice is seeking experienced RN to join our Leadership Team. Working under the supervision of the Regional Clinical Director, the Patient Care Manager is responsible for administering the clinical aspects of our hospice program, for ensuring the provision of quality care to patients and for continued evaluation of services and staff. Rivercross Hospice is a locally owned Hospice with offices in Bartlesville, Tulsa, and Muskogee and provide services to the surrounding area's. The Patient Care Manager must have a valid RN License to be eligible for this position. The Patient Care Manager is responsible for providing direct team leadership and direction of hospice services. The Patient Care Manager implements goals and objectives for hospice services that meet and promote the standards of quality and contribute to the total organization and philosophy.

Essential Job Functions/Responsibilities

Coordinate all aspects of patient care

Supervising and/or coordinating activities of all patient care

Liaison to Hospice Medical Director and Hospice Physicians

Provides orientation, teaching and guidance to Nursing Staff and hospice aides, and other staff where needed, to improve quality and quantity of service

Evaluating performance of patient care team

Leads IDG with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement

Assists in the screening and interviewing of new clinical team employees

Assisting Administrator and other supervisory hospice personnel in the planning, implementation and evaluation of in-services and continuing education programs

Provides ongoing training to care team staff and supports them by completing ride along visits

Promotes customer service orientation to all hospice personnel

Ensures proper documentation for billing purposes is in the EMR and participates in the triple check process.

Participate actively in quality assessment performance improvement teams and activities

Manages patient care staff budget and productivity including the budget for all medical supplies, including DME and pharmacy supplies.

Facility chart reviews

After hours On-Call and back-up nursing duties as needed, including Admissions

Perform other duties and activities as delegated by the Administrator.

Position Qualifications

Current Nursing license, Preferably BSN.

Two years supervisory or management and leadership experience preferred.

Ability to manage across multiple disciplines.

Excellent critical thinking skills.

Team development skills to work as part of the organization’s management team.

A passion for working with Hospice and helping patients and families to have a quality end of life experience.

Excellent clinical, end of life case management skills, with a strong background in comfort care.

Excellent observation skills and good nursing judgment.

Excellent verbal and written communication skills.

Licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.

Current CPR certification
