Rising Star Homecare Services is Hiring a Care Coordinator Near Edgewood, MD

Rising Star Homecare Services is seeking a motivated, self-driven and compassionate individual to fill the position of Care Coordinator. Working under the supervision of, and collaboratively with the Administrator, the Caregiver Coordinator will be responsible for organizing work schedules onboarding for caregivers to match their availability with the needs of the clients in care to ensure effective and seamless delivery of quality services. The care coordinator prepares and maintains weekly schedules of services for clients and allocates the caregivers to clients per need. The position requires the employee is on the phone up to 50% of their work shift and complete other essential function on an as-needed basis.

Location: Rising Star Homecare Office, located at: 1018 Edgewood Rd, Edgewood, Harford County

Responsibilities:

· Provide empathetic telephonic customer service for Rising Star Homecare Serves clients.

· Actively listen and probe patients in a professional and timely manner to understand and evaluate patient’s need, specifically whether the need is clinical or administrative.

· Coordinates and directs in-coming calls to relevant staff for and escalates situations as needed.

· Communicate and collaborate with members of the Rising Star Homecare Serves team.

· Documents emergent patient interactions across various communication methods

· Manage the scheduling process for patients wanting to schedule new visits or reschedule existing ones.

· Work collaboratively with the Administrator and Director of Nursing to ensure the employees and client files are up to date (i.e. all the relevant documents are in place)

· Supports management with the hiring of caregivers (i.e. placing adverts, scheduling interviews, etc.)

· Performs other ad hoc administrative tasks as assigned.

Work Orientation & Experience:

At least 2 years relevant work experience in the healthcare field required; Experience in utilization management, quality assurance, home care, community health, long term care, medical assistant or occupational health required. Managed care experience preferred.

Skills & Abilities:

Good customer service skills; work requires the ability to understand and follow oral and written instructions. Ability to organize multiple tasks and manage time effectively; works independently. Gathering and reviewing information from various sources. Communicating effectively with the patient and with other healthcare providers. Use of computer hardware and software, including but not limited to word-processing and spreadsheets.

Job Type: Full-time

Pay: $19.00 - $22.00 per hour
