Shift: Monday - Friday, 9:00 AM - 5:00 PM

Geriatric Care Manager (RN) Summary:

We are looking to improve the quality of our in-home, senior care agency with a Geriatric Care Manager (RN). We are seeking a candidate who is passionate about improving the lives of seniors or differently abled people in our community. As a Geriatric Care Manager (RN), you will be helping our clients live a safer, more congenial life from the comfort of their home, through client-facing responsibilities and Caregiver education and management.

The primary focuses of this role will include:

• Working with clients and their families to identify, plan, and oversee the care of themselves or their loved ones

• Enriching the education and knowledge of our Caregivers to help them provide the highest level of care

Geriatric Care Manager (RN)Responsibilities:

• Manage care plans for clients - within a Class B, non-medical agency

• Perform in-home supervisory visits with clients to update care plans (as needed)

• Ensure exceptional quality care for clients by providing Caregivers with the best possible oversight

• Assist with Caregiver's continuing education classes

• Train incoming Caregivers in onboarding training sessions

• Perform initial in-home assessments for clients (as needed)

• Help to effectively determine and meet client care needs

Geriatric Care Manager (RN)Requirements:

• Current nursing (RN) license that allows you to practice in Colorado

• Associate Degree in Nursing (ADN), or a Bachelor of Science in Nursing (BSN) from an Accredited Nursing School

• Valid Driver’s License

• Vehicle and Auto Insurance, to provide supervisory visits and in-home assessments

• Vaccination for COVID-19, or written exemption

Geriatric Care Manager (RN) Preferred Skills:

• Ability to communicate openly and professionally with clients and coworkers

• Positive attitude and excellent interpersonal skills

• Ability to read, write, speak, and understand English, as needed for the job
