Resilience Recovery Resources is Hiring a Case Manager Near North Palm Beach, FL

Resilience Recovery Resources is an all male, young adult aftercare treatment program with outpatient substance abuse treatment, which works in conjunction with our sober living facility, Resilience Recovery Residences. Clients are typically 16-26 and transition to us after completing a residential treatment or detox program.

Case Managers work as part of the clinical team to ensure the best care for all clients. They work under the direction of the Clinical Director to collaborate with each client’s Primary Therapist in developing the best treatment plan for their care. They meet with client’s one-on-one to provide guidance and direction, as well as act as the liaison between the clients’ families and their treatment team, which may include additional psychiatrists, specialty therapists, and other doctors and professionals. They are each families’ main contact person (in addition to their Primary Therapist), while their loved one is in treatment under our care. They provide necessary insight to the rest of the team about each client’s programmatic progress and provide their recommendations for their care to Senior Management. Case Managers have a lot of autonomy in terms of the clinical/programmatic decisions they make for their cases such as whether or not they believe the client is ready to “level up,” what restrictions should be imposed, etc.; while being in complete communication with the rest of the team and getting support from Senior Management where appropriate. Case Managers employ Motivational Interviewing techniques and evidence-based practices as outlined in our Policies & Procedures. It is important for Case Managers to develop a therapeutic alliance in order to best support each client as they navigate the challenges of their recovery journey. This position requires individuals who are level-headed, can hold appropriate boundaries, think on their feet, and have a great rapport with clients and a high level of communication skills. They are committed members of the clinical team, looking to further their careers in this field.

Daily Duties

  • Maintain case load of up to 20 clients
  • Provide client orientation to treatment
  • Complete initial Case Management session, identify goals and client needs (such as outside referrals)
  • Work under supervision of the Clinical Director and with the Senior Management team and each client’s Primary Therapist to create treatment plans for each client
  • Assist in clinical evaluation including using evidence-based tools (ASAM, DSM V criteria) for appropriateness of continued treatment
  • Oversee client assessments are completed in a timely manner including BAM and WHODAS; client HIV pre-test, education, exam, and provide post-test counseling
  • Work under supervision of the Clinical Director and with the Senior Management team to create treatment plans for each client
  • Meet with each client on a weekly basis to develop attainable goals, review their progress and accountability from the week and how they are showing up in the community
  • Provide structure, support, and proper motivation to their caseload. Employ counseling techniques to develop a therapeutic alliance with each client; provide structure, support, and proper motivation to their caseload
  • Work with Primary Therapist to assess client progress throughout their stay, identifying barriers to treatment and work on overcoming those barriers
  • Assess client readiness and motivation for treatment; identify risk factors and present to clinical team
  • Assess signs and symptoms of relapse behavior; observe stages of recovery and work with Primary Therapist to provide relapse prevention tools
  • Act as liaison and provide weekly updates to client families and/or referring parties and other members of their treatment team if applicable (i.e. doctors, probation officers, outside case managers, therapists, or psychiatrists, etc.)
  • Obtain appropriate ROI’s, provide outside referrals, and and create outside appointments as needed
  • Complete weekly Case Management and Family Call documentation
  • Meet weekly with clinical team for clinical meetings and supervision as needed; keep team informed of client progress, challenges, and at-risk behaviors; troubleshoot challenging clients with Senior Management
  • Field family calls and any interventions as needed, while holding appropriate boundaries
  • Conduct educational groups and group therapy as directed
  • Be present for all indicated Staff Meetings and any House Meetings during their shift

Case Manager Qualifications

Education

  • Degree in social work, psychology, or a related human services field (or currently pursuing)

Experience

  • 1 year of work experience in the substance abuse and mental health field
  • Experience working with adults and adolescents with substance abuse or mental health concerns
  • Must have experience with KIPU systems or an equivalent client care system
  • Must have at least 1 year of sobriety (if in recovery)
  • Must have a vehicle, valid driver’s license, and car insurance
  • Must have great communication, leadership, organizational skills, and ability to hold boundaries

Knowledge

  • Knowledge of state and federal statutes regarding client confidentiality laws
  • Knowledge of workplace violence
  • Knowledge of corporate integrity and compliance program
  • Knowledge of substance abuse and mental health, and the related concerns that individuals with substance abuse and co-occurring disorders face

Required Screening

  • Completion of a urinalysis drug screening
  • Completion of a local criminal background check and Level 2 background check with no disqualifying offenses as per Affidavit of Good Moral Character

Job Type: Full-time

Pay: $40,000.00 - $45,000.00 per year
