Queens Community House is Hiring a Case Manager Near Forest, NY

We are seeking a qualified Case Manager with a strong commitment to social services and working with older adults. Case Management services help older persons with functional impairments gain access to appropriate services, benefits, and entitlements needed to age safely at home and maintain their quality of life. Case Managers do so by developing trusting relationships with the clients and their caregivers and engaging them in a collaborative process of problem-solving. 

The Case Manager has responsibility for managing a caseload of 65 clients and will provide case management support for these clients consistent with the goals of being professional, timely, culturally aware, and sensitive, and engaging the clients in achieving maximum potential as defined by their abilities. Primary Responsibilities:

  • Screen new potential case management clients through the intake process
  • Provide clients with accurate and appropriate information on services, benefits, and entitlements 
  • Conduct in-home client assessments and re-assessments 
  • Develop care plans consistent with client needs that reflect culturally sensitive options that make efficient use of community resources
  • Serve as a liaison with external services and organizations
  • Arrange for the provision of services, advocate, and intervene with other agencies on behalf of the clients 
  • Compile and maintain complete and accurate case records 
  • Assist and support clients with the need for utilization of Supplemental Services
  • Participate in Department for the Aging (DFTA) and other in-house or external training to ensure the completion of 16 hours of yearly training 
  • Participate in Staff Development activities and QCH’s annual Staff Development Day and attend regular staff meetings
  • Work as a collaborative and effective member of the case management and QCH staff team
  • Other duties as assigned

Qualifications:

  • BSW or related bachelor’s degree in social services or public health
  • 1-2 years of experience working with the older adult population 
  • Knowledgeable about and sensitive to the diverse needs, preferences, and characteristics of older persons in the program’s catchment areas
  • Knowledgeable about the communication needs of persons with visual and/or hearing impairments 
  • Comfortable with, and able to multitask 
  • Ability to use various computer applications, software, etc.
  • Bilingual in Spanish

PREFERRED skills/experience/qualities:

  • MSW degree or Master’s level degree in a related field
  • Bilingual in, Mandarin, Cantonese, Russian
  • Knowledge of the Senior Tracking, Analysis, and Reporting System (STARS) database

Status: Full-Time, Exempt 
Compensation: $59,000 annually
Schedule: 35 hours per week, Monday - Friday
