The Puller Veterans Care Center in Warrenton, Virginia is in search of an experienced Director of Rehabilitation! This is a state position with excellent healthcare and retirement benefits.

The Puller Veterans Care Center is a state-of-the-art facility, luxury skilled nursing facility that was built to serve our Veterans. They served us, now it is our duty to serve them. Our goal is to provide each resident with a meaningful life through a whole-person approach to care. We provide physical healthcare and recreational, spiritual, therapeutic, and social opportunities that contribute to overall physical and mental health.

Job Duties

Clinical Rehab Services:

  • Plans, directs, and coordinates comprehensive rehabilitation services
  • Modifies treatment plans to comply with changes in clients' status
  • Identifies environmental impediments to client or patient progress through interviews and review of patient records
  • Monitors, evaluates, and records client progress according to measurable goals described in treatment and care plan
  • Assures policies and procedures are being adhered to for the safety and best treatment of residents
  • Manages caseload allotment and need for additional per-diem or permanent staff on a daily basis.

Assessment, Treatment Plans, and Transitional Care:

  • Works with the IDCP team to develop a care plan individually designed to assist the resident in attaining the highest practicable physical, mental and psycho-social functioning
  • Assists in developing needs statement to include resident’s contributing strengths and weaknesses and any risk factors
  • Plans and coordinates client or patient care and rehabilitation, following through to ensure service efficacy
  • Develops interim care plan by the ARD after admission and as required in accordance with current policy
  • Assists IDCP team in the coordination of discharge planning

Supervision & Strategic Planning:

  • Plans, coordinates, executes, and supervises all daily activities of the Department of Rehabilitation as to meet federal, state, and facility policies, procedures, and regulations
  • Creates long-term work schedules to maximize departmental outcomes utilizing the appropriate levels of staffing and assigning specific duties
  • Establishes and implements goals, objectives, operational standards, and procedures for the department
  • Supervises the daily activities of staff to ensure assigned work is completed in a prompt, accurate manner
  • Motivates, develops, and directs staff as they work, identifying the best people for the job
  • Monitors and assesses performance of staff to make improvements or take corrective actions
  • Confers with staff to resolve performance and personnel problems, and to discuss facility/state policies
  • Addresses complaints and/or concerns from staff, residents or families from all areas
  • Investigates complaints and takes appropriate corrective action as necessary
  • Coordinates activities with other departments to ensure that services are provided in an efficient and timely manner
  • Determines staffing requirements, and interviews, hires, and trains new employees or oversees these processes to ensure successful staffing
  • Directs and coordinates department’s financial and budget activities to maximize efficiency
  • Maintains and reviews required records of work hours, budgets, payrolls/KRONOS, and other information as required
  • Reports any hazardous conditions, equipment, accidents, and incidents
  • Locates, selects, and procures equipment and materials for the department
  • Ensures equipment is checked and maintained
  • Follows procurement policies and processes

Performance Management:

  • Ensures expectations are clear, well communicated, and relate to the goals and objectives of the department or unit
  • Provides staff with frequent, constructive feedback, including interim evaluations and in-service training sessions as appropriate
  • Ensures staff has the necessary knowledge, skills, and abilities to accomplish goals. Addresses and documents performance issues as they occur
  • Reviews and communicates safety issues to ensure a safe and healthy workplace

Special Assignments:

  • May be required to perform other duties as assigned
  • May be required to assist the agency or state government generally in the event of an emergency declaration by the Governor

Minimum Qualifications

  • Completion of requirements for minimally a Bachelor’s (preferably Master’s) Degree in physical, occupational or speech therapy from an accredited education program and must be licensed or eligible to practice in Virginia as a physical, occupational or speech therapist

Managerial experience in a long term care setting is necessary

Experience with neurologically involved, developmentally disabled patients is beneficial. Previous professional experience, as a treating therapist, in a long term care or adult geriatric setting preferred

Preferred Qualifications:

  • Speech Language Pathologist Preferred
