Title: Case Manager/Career Coach Status:
HOURS: Monday - Friday
Pay $17-22/ hour
Non-Exempt Reports to: Director of Clinical Services
SUMMARY OF RESPONSIBILITIES
The Case Manager/Career Coach is responsible for assessing client’s needs for resources and services on an outpatient basis, coordinating supportive services, and providing career readiness training, consultation and support to ensure that Project LIFT program participants receive maximum benefit from Project LIFT’s unique combination of substance abuse and mental health treatment and vocational training.
SPECIFIC DUTIES AND RESPONSIBILITIES:
The specific duties will include:
· Performing needs assessments to determine each client’s level of physical and mental functioning, needs for support and services, and areas to target for improvement.
· Assisting with program intake and client/family interview processes as needed.
· Assisting in coordination with community resources and maintaining relationships with community service providers.
· Assisting with development and delivery of vocational readiness curriculum.
· Providing guidance to program participants regarding job readiness skills, job searching, interviewing techniques and follow-up.
· Assisting clients in the creation/editing of resumes and cover letters.
· Developing and deliver group counseling modules to assist program participants in adopting skills needed for success in the workplace, including time management, effective communication, anger management and assertiveness, etc.
· Developing and maintaining relationships with local businesses and potential employers, including developing and following through with internship and other mentoring programs.
· Assist with discharge planning and coordination as necessary to support clients’ needs for additional treatment and services.
· Assisting with implementing the program evaluation framework to assess the strengths of the program and identify areas of improvement.
· Assist with the delivery of services as needed to increase effectiveness and efficiency. This may include assisting with meal preparation, providing transportation for enrolled clients, facilitating relationships with mentors, etc
· Perform other duties as determined by the Director of Clinical Services.
KNOWLEDGE, SKILLS AND ABILITIES:
QUALIFICATIONS:
Bachelor’s degree or higher in mental health, social work, criminal justice, or related field. Associate’s degree with CAC or CAP may be considered.
PHYSICAL ACTIVITY/WORKING CONDITIONS:
While performing the duties of this job, the employee is regularly required to sit, talk and hear. The employee is frequently required to use hands to finger, handle, or feel and reach with hands and arms. The employee is also required to sit at a desk and use a computer. The employee is occasionally required to stand and walk. Specific vision abilities required by this job include close vision and ability to adjust focus. The noise level in the work environment varies. In addition, the employee must be able to lift up to a minimum of 25lbs, able to work in non-air conditioned environment and to stay on feet for extended periods of time. The physical demands and work environment described is representative of those that must be met by an employee to successfully perform the essential functions of this job.
