Presbyterian Medical Services is Hiring a Care Coordinator II Near Albuquerque, NM

Job Description
This position may qualify for hybrid-remote options after 90 day probationary period.
Under supervision, this position assists the Integrated Care Team (Primary Care Physician, Nurse/Medical Assistant, Behavioral Health provider, etc.) by proactively coordinating care for patients to facilitate the achievement of quality and cost effective health outcomes.
Qualifications
REQUIREMENTS
EDUCATION AND/OR EXPERIENCE:
  • Associate's degree AND five year's relevant healthcare experience; OR
  • Bachelor's degree AND four year's relevant healthcare experience; OR
  • Master's degree AND two year's relevant healthcare experience. Self-starter with strong verbal and written communication skills.
  • Comfort with change and quality improvement processes.
  • Experience with Electronic Health Records strongly preferred.
  • Experience with Motivational Interviewing strongly preferred.
  • Knowledge of New Mexico and local community resources strongly preferred.
  • Bilingual (English / Spanish) or (English / Navajo) preferred.
CERTIFICATES, LICENSES, REGISTRATIONS:
  • Certified Case Manager (CCM) preferred.
SCREENING REQUIREMENTS:
This position requires successful completion of an initial post-offer of employment:
  • Criminal Records Check.
  • According to PMS Human Resources Policy 205.006, "Healthcare Personnel Testing & Immunization Requirements," this position must comply with required testing and immunizations. Testing and immunizations including TB, Hepatitis B (consent or waiver), Tdap, MMR, and Varicella, will be available at the employee's PMS health center. Influenza vaccines will be required according to dates set by the CDC and NMDOH.

Presbyterian Medical Services (PMS) is the successor to the United Presbyterian Church's Medical Mission work in the Southwest, which began in 1901.
