Key Responsibilities
- Collaborate effectively with both therapy and facility teams to deliver exceptional care.
- Provide education and support to family members and caregivers regarding therapy processes.
- Participate in facility training sessions and in-service programs to promote ongoing learning.
- Attend care plan meetings to discuss resident progress and contribute to treatment strategies.
- Maintain accurate daily documentation of resident care activities.
- Work closely with the supervising Physical Therapist to ensure comprehensive and coordinated care.
- Strong interpersonal and communication abilities to foster effective collaboration.
- Proficient understanding of physical therapy principles and practices.
- Competent in daily documentation and compliance with healthcare standards.
- Valid Physical Therapist Assistant licensure in Pennsylvania.
- Previous experience in a skilled nursing environment is preferred but not mandatory.
- A genuine passion for improving the quality of life for residents through dedicated care.
- Associate degree from an accredited Physical Therapist Assistant program.
