Please note this position is part of a partnership between Preferred Family Healthcare/Clarity Healthcare and Jordan Valley Community Health Center. If you are hired for this position, you will be considered an employee of Jordan Valley Community Health Center. As a result, your compensation and benefits eligibility will be determined by Jordan Valley Community Health Center.

Your role:

As a care coordinator you will Determine eligibility of persons applying to receive assistance from government programs and clinic resources, and provide support for services needed.

You will:

• Supports the development and implementation of the physician aligned case management model.
• Deals discreetly with sensitive confidential information contained in minutes, letters, reports, and other documents.
• Is responsible for department’s operational excellence and ensures that the department delivers quality services in accordance with applicable policies, procedures, and professional standards.
• Meet with new and established Medicaid patients to answer questions and provide information about the clinics and services.
• Assist patients who have difficulties completing paperwork or understanding information.

We think you’d be great for this role if you have:

• Three (3) years minimum clinic experience in case management or comparable position.
• Bachelor’s Degree, preferred.
• COVID Vaccine required for all new employees
